Dear Kevin:
I am the attorney who was retained by Riverview Hospital in regard to the above matter,
Riverview Hospital did obtain amended ADLS approval for revised building elevations for both
buildings to be situated on Parcel 1 of tl1e Riverview Medical Park. This amended ADLS
approval occurred at the August 3, 2004, Special Studies Conunittee Meeting of the City of
, Carmel Plan Connnission. The Plan Commission will send to me in the next few days a Letter
of Grant. When I receive it~ I will send it to my contact at Riverview Hospital, Ja.e Ebert, Vice
President of Support Services. In the meantime, should you have any questions regarding the
approval as it relates to your meeting with Jeff Kendall, you should contact Jon Dobosiewicz
(571-2417), who is a planning administrator at the City of Cannel. Jon can confirm the above
and also answer any other additional questions Jeff Kendall may have regarding the approval.

TYPE OF EVALUATION: Initial X Routine 0 Follow-up 0 Complaint
This evaluation is intended to assess the level of compliance with 327 lAC 15-5 (Rule 5). It is also intended to identify areas where
additional measures may be required to control erosion and sedimentation. All practices recommended in this report should be
evaluated as to their feasibility by a qualified individual with structural practices designed by a qualified engineer.
All erosion and sediment control measures shall meet the design criteria, standards, and specifications outlined in the "Indiana
Handbookfor Erosion Control in Developing Areas" or similar guidance documents.
Current Site Information: Pond at the east end of site, the stone base for the roads and the stormsewer are
installed. One or two building sites have been started but construction activity has stopped. Weeds have taken
over much of the site.
The Following Items Have Been Evaluated and Assigned a Designation of:

Install an appropriate sediment control practice between construction areas and lower lying areas.
Replace/Repair silt fence and/or straw bale barriers.
Entrench silt fence (6-8 in.) and/or straw bale barriers (4 -6 in.) and stake straw bales.
Remove accumulated sediment from:
o sediment traps/basins,
o behind silt fence/straw bales,
o around storm drain inlet protection devices,
o streets and gutters (Do not flush with water)
X Temporary seed, fertilize, and/or mulch: Disturbed areas left idle over 15 days must be
stabilized with grass, straw mulch or equivalent cover.
Permanent seed, fertilize, and mulch areas that are at final grade
Protect storm drain inlets:
o curb inlets,
o drop inlets
Reshape and stabilize sideslopes of :
o sediment traps/basins,

Reshape and stabilize conveyance channels:
Place greater emphasis on erosion and sediment control on building sites; contractors,
subcontractors, material vendors, and others should be made aware of erosion and sediment control
requirements. Appropriate perimeter sediment controls (e.g. silt fence) and stable construction
entrances shall be utilized on all sites. Sediment tracked into the street shall be removed (do not
flush with water) at the end of each work day.
Other Requirements:

Enclosed please find the following:
1. Eight(8) informational brochures for distribution to the Special Studies Committee members; and
2. A filing fee check in the amount of $500.00.
Jae Ebert is in the process of obtaining the signed Owner's Consent from Bryan Chander of Plum Creek
Partners. As soon as we obtain the Consent, we will file it with your office. Jae is also in the process of providing to
me the last approved site plan and the revised site plan which will incorporate the changes desired via the amended
ADLS application and which will show the new building footprints on the site. Finally, I will submit to you next
week an area map showing the surrounding zoning of the adjacent parcels. With respect to your question what
sections of the PUD Ordinance we are deviating from, to my knowledge, we are not deviating from any of the
provisions of the approved PUD Ordinance. Instead, these are slight revisions and modifications to the approved
building, elevations which are within the spirit of the approved PUD Ordinance and are designed to improve the
functionality of each building.

Changes in Riverview Hazel Dell Project Design
Building 'A' Primary Care Building (North Building):
. Reducing drive-throughs from a double drive-through to a single drive-
through.
. Lowering the entry way roof peak to the same height as the rest of the
building and keeping the chimney height in proportion to the roof line.
. Partial interior space will be shell space for future physicians.
. Removing one false gable on South elevation.
Building 'B'Rehab Building (South Building):
. Moved second floor occupied space to first floor.
. Expanded square footage to just slightly less than 20,000 square feet.
· Squared building footprint to facilitate structural truss system.
· From original design, added drive up canopy and from second approved
design, changed canopy gable.
· Removed windows from second story attic space.
· Changed from brick gable to efface gable (East elevation).
· Changed gable and glass block configuration on West elevation.
