McBride City Engineer Date: /2/0(/20/2 AUMAN AND NEWARK NEIGHBORHOOD IMPROVEMENTS PHASE 2 NAPPANEE DRIVE SIDEWALK & DRAINAGE IMPROVEMENTS 09-06 CHANCE ORDER #1 Item No. Item 4 B-Borrow for Structural Backfill overrun because sand used to backfill an existing sanitary sewer on Nappanee Drive would collapse into the new trench excavated for the storm sewer. This unforeseen condition increased the size the trench and caused more#8 stone to be used for backfill. 5 Flowable Backfill for Pipes overrun because sand used to backfill an existing sanitary sewer on Nappanee Drive would collapse into the new trench excavated for the storm sewer. This unforeseen condition increased the size the trench and caused more flowable fill to be used for backfill. There was also an error in the plan quantity calculations for the trench excavated to lay pipe across Range Line Road 7 Storm Sewer Pipe, 15" Class II RCP overrun because an additional foot of pipe was required to make a secure connection to an existing structure. 11 Drain tile, PVC, Undistributed overrun because existing underground drain pipes were found during excavation. The existing drain pipes were connected to the new storm sewers to maintain drainage from existing homes and businesses. 18 Inlet J-10 overrun because an additional inlet was required at the intersection of Range Line Road and Nappanee Dr. to provide drainage for a low area behind the sidewalk. 19 Flowable Fill (Concrete A Concrete for Patch) underrun because the thickness of the patches on Range Line Road and Nappanee Drive were less than shown on the plans. More Flowable Backfill (Item #10) was used in its place. 21 Curb Ramps with Truncated Domes overrun because of error an in the plan quantity. Additional ramp area was required because of existing field conditions. 22 4" Concrete Sidewalk - underrun because of an error in the plan quantity. Additional ramp area was required because of existing field conditions. 23 Concrete Curb - underrun because of an error in the plan quantity. Less concrete curb was required because of existing field conditions 24 24" Combined Concrete Curb & Gutter overrun because a portion of the curb had to be repoured to allow for a wheelchair ramp. Final quantity was substantially as planned. 25 Concrete Pvm't. for Commercial Drive, 6" overrun because of existing field conditions required more concrete pavement. Final quantity was substantially as planned. 26 Concrete Pvm't. for Commercial Drive, 9 overrun because of existing field conditions required more concrete pavement. Final quantity was substantially as planned. 28 Unsuitable Materials, (Undistributed) underrun because no unsuitable material was encountered. 29 Compacted Aggregate, 53, Base overrun. Final quantity was substantially as planned. 30 HMA Surface, A, for Road Patching underrun because the item was eliminated by EWA 5, HMA Patching. 31 Tack Coat underrun because the item was eliminated by EWA 5, HMA Patching. 39 Nursery Sodding for Lawns underrun because the item was eliminated by EWA 2, Erosion Control Blanket (Rangeline) and EWA 3, Mulch Seeding (Nappanee). 40 Water underrun because the item was eliminated by EWA 2, Erosion Control Blanket (Rangeline) and EWA 3, Mulch Seeding (Nappanee). EWA 1 Mobilization/ Demob. For Seeding overrun because (Item #39 Nursery Sodding for Lawns) was eliminated from the contract because of drought conditions. The area was seeded. EWA 2 Erosion Control Blanket (Rangeline Rd.) overrun because (Item #39 Nursery Sodding for Lawns) was eliminated from the contract because of drought conditions. The area was seeded. EWA 3 Mulch Seeding (Nappanee Dr.) overrun because (Item #39 Nursery Sodding for Lawns) was eliminated from the contract because of drought conditions. The area was seeded. EWA 4 Mobilization/Demob. for Asphalt Patching overrun because additional pavement was removed on Nappanee Drive due to granular backfill material over the existing sanitary sewer fell into the trench excavation for the new storm. Additional asphalt material was required to patch over the existing sanitary sewer line. EWA 5 HMA Patching overrun because additional pavement was removed on Nappanee Drive due to granular backfill material over the existing sanitary sewer fell into the trench excavation for the new storm. The pay item B-Borrow for Structural Backfill overran because the stone used to backfill an existing sanitary sewer under Nappanee Drive collapsed into the new trench excavation for the storm sewer during pipe laying operations. Also, portions of an old buried storm sewer were encountered along the edge of the roadway on Nappanee Drive. These two unforeseen conditions increased the size of the trench and caused undermining of existing pavement. This increased the amount of#8 stone required to backfill the trench by 64%. The item overran $81,275.76. Two new pay items were made Mobilization & Demobilization for Asphalt Patch and HMA Patching to make the necessary repairs to the roadway. This work included cutting and removing existing pavement and using HMA asphalt patch material and HMA surface to repair the patched areas. The cost to make the pavement repairs was $35,817.86.
