construction until all issues 'have been resolved. All Carmel Utility issues must also reresolved. Thedesign
engineer must certify all dra\vings submitted for final approval. This office will require a minimum of four-sets
(nlaximum of six-sets) of approved dra\vings after all issues have been resolved. The dra\yings will be stamped
approved and signed by both the City Engineer and Director Carmel Utilities. The Developer/Owner \vill receive
one~set \vhich is to be maintained on the construction site at all times. Carmel Utilities will receive one-set. Our

Engineering. If more than the minimum of four-sets are requested to be approved, the additional sets (maximum
total of two additional) must be submitted with the four required sets.
6. Please be advised that any installation of signs, walls, irrigation systems, etc. within dedicated easements and/or
right of way will require Consent to Encroach approval.
7. Carmel Utilities does not subscribe to "Holey Moley" and should be contacted directly for all sanitary sewer main
locations.
8. Carmel Utilities will provide a separate review of this project for sanitary sewer issues. Please assure that Carmel
Utilities receives copies of the construction plans including all revised plans.

BONDING REQUIREl\'IENTS
10. Upon initial review, it appears the following bonding requirements may apply to this project:
Performance/Maintenance Guarantees
A Performance Guarantee will be required for the sidewalks in the Pennsylvania Street right of way. We require a
Certified Engineer's Estimate for 100% of labor and materials required for the construction. The Performance
Guarantee amount is based upon the Engineer's Estimate. When the construction has been completed and the
Performance Guarantee has been released, a 3-year, 10% Maintenance Guarantee will be required.
Street CutlRight of Way Permit and Bonding Requirements
Any work in the dedicated right of way of Pennsylvania Street and/or Carmel Drive will require a Right of Way
Permit and a License and Permit Bond. The bond amount will be determined by the number of instances of work
in the right of way @ $2,000.00 per instance. Open pavement cuts in dedicated streets require approval by our
Board of Public Works and Safety. Please contact our Right of Way Manager/Inspector, Fred Glaser, to arrange
permitting and bonding.
PROJECT COMMENTS-GENERAL
11. We have not had a written response to our original review letter or revised plans for Phase 1 of this project. What
is the planned construction schedule for Phases 1 and 2?
12. We have engaged the services of Crossroad Engineers, PC to provide storm drainage review. The drainage
calculations have been forwarded to Crossroad. We will provide their review as soon as it is available.
13. Please advise the Owner and Contractors involved in this project that the City of Carmel has a strict ordinance
regarding mud and other substances deposited on City,streets by vehicles and equipment used in excavation,
construction, demolition or otherwise within the City Limits. Be prepared to clean Pennsylvania Street during the
duration of this project.
PROJECT COMMENTS-CONSTRUCTION PLANS
14. Sheet CIaO-Existing Conditions Map
. Label and dimension the Pennsylvania Street right of way line (existing and proposed if different) on this and
all sheets. We would expect dedication of additional right of way if the existing does not meet the
requirements of the Thoroughfare Plan, which is 90' (45' half).
15. Sheet C200-Site Plan
. Why are the drive aisles only 22' wide?
. Alternative Transportation Plan calls for a 10' wide multi-use path on Pennsylvania Street.
We reserve the right to provide additional comments as this project progresses through the approval process and receipt
of the storm drainage plan review by Crossroad Engineers.
The above comments are based upon the Engineering Department's initial review. Please provide our office with a"
follow up letter regarding this review and revised plans, indicating all revisions. A second review will focus only on
detailed construction plans. It is critical that our office be made aware of all modifications made on the plans being re-
submitted, particularly if any such changes are considered "new" or fall outside of our initial review. If you have

To Whom It May Concern:
This letter serves to acknowledge receipt of a transmittal received 8/4/04 containing the
plans for the above-mentioned project. After reviewing the plans the Highway
Department has the following comments:
1. In accordance with the Hamilton County Thoroughfare Plan and the 96th Street
'Corridor Study, additional right of way is required 'at the intersection of 96th Street
and Haverstick Road. On Haverstick Road, the right of way from the intersection
toa point 500' to the north is a 60' half. From that point to the northern property
line the right of way is 40'. On 96th Street, the rightof way is a 75' half. A 50'
triangular cut is also needed at the intersection, connecting the right of way for
, the perimeter streets. The'corner cut is also needed at the intersection with Wild
Cherry Lane.
2. The right of way should be labeled and dimensioned on the Site Plans.
3. A Major Commercial Driveway permit will be required for the construction of the
entrance. Please contact Mr. Robb Chadwell, Highway Department Permit
Inspector, for the necessary forms and information.
4. Permits will be required for all utility work done within the right of way.
5. Please dimension the accelldecellanes. The dimensions for the lanes should be
100' taper and 150' lane. Additional right of way may need to be acquired north
of the site to accommodate the lengths of the lanes.
6. The right turn lane on Haverstick Road should be constructed from the north
property line to the intersection with 96th Street. A 50' radius should also be
constructed atthe intersection of 96th and Haverstick Road.
7. A left turn lane will need to be developed on 96th Street at Haverstick Road. The
necessary right of way may exist on the south side of 96th Street, but will need to
be researched in the Marion County records. If the necessary right of way is not
present, it will need to be acquired prior to approval of the permits.
8. Will the property be platted?
9. A roadside drainage swale will be needed around the entire perimeter of the site.
10. Where will the site drainage be discharged? The drainage will not be permitted to
enter the right of way.

11. Please note that decorative signs, sprinkler systems, trees, landscaping mounds,
fences, light poles or other such amenities are not permitted in the right of way.
12. It appears that some of the landscaping shown will be within the 75' right of way.
Please remove the trees from the right of way.

LETTER OF APPROVAL
The undersigned has reviewed the proposed plans for
Kite Medical Office Complex - Phase II
and has approved the plans subject to the following:
1. Our office is requesting a meeting to discuss the location of the fire department connection and
the request of an additional fire hydrant for this project. We are also requesting an exterior
entrance to the fire sprinkler riser room.
2. We are requesting that a Knox box for fire department emergency access and Knox fire
department connection caps be provided. Knox products and accessories can be purchased on
line at www.knoxbox.com.
3. What is the height of the building and will the building have a basement?
4. Will the building have a fire alarm system? If so we are requesting a remote enunciator be
located at the main entrance of the building.

This letter is in response to your Development Plan and ADLS Amendment Applications for Kite Medical
Office Complex, Phase 2. This is a preliminary letter to let you begin on revisions. Members at the TAC
Meeting will make additional comments on August 18. Please complete the following items after you have
received additional comments at the T AC Meeting.
1. Please label the dimensions of the building, on the site plan.
2. Please show/label the 90-ft build-to line from the US 31 Right-of-way.
3. Please show/label the 20-ft build-to line from the Pennsylvania St. Right-of-way.
4. Please show/label the proposed and existing half rights-of-way and label the widths for US
Highway 31.
5. Please provide a list of adj acent property owners, certified by Hamilton County Auditor's Office.
6. Please present material and color samples to the Plan Commission Meeting.
7. Provide a signage location plan (site plan).
8. Provide signage details, materials, square footage, dimensions, lighting of, etc.
9. Please forward a copy of Urban Forester's Approvalletter/e-mail.
10. Show/label the landscape buffer yards, per Chapter 26.04.
11. Please provide light fixture cut sheets/details.
12. Please provide trash enclosure details.
13. Provide black-line building elevations, which show screening of rooftop-mounted equipment.
14. The building elevations appear satisfactory, but please show that they meet the Golden Section.

We have reviewed the construction plans submitted to the Hamilton County Surveyor's
Office on August 4~ 2004 for this project and have the following comments:
1. This project falls within the jurisdiction of the City of Carmel.
2. This proposed site DOES NOT fall in a Cannel Well Head Protection Area.
3. Please coordinate the storm sewer design with both the City of Carmel
Engineering Department and this office.
4. This proposed project falls in the W.R. Fertig Drainage Sheet
5. The W.R. Fertig Watershed has an allowable release rate of 0.25 cfs per acre. It
appears from the drainage report that the Hamilton Crossing drainage report set
the restriction for this site slightly lower, The drainage calculations provided do
comply with the allowable release rate.
6. Please submit an outlet permit, for the indirect discharge into the W.Rp Fertig
Regulated Drain, to this office. The application is available on our website at
http~I/\VWW.co.hamilton.in.us, go to Departments, Swveyor, and Forms.
7. The HCSO would recommend you show the emergency spillway in the grading
contours on sheet C300 and also show a cross section detail.
