Dear City of Carmel,
This etter s n reference to the build-out design for 40 N. Range ine. There will be 7 rooms on the first
floor which will contain e eCtdca equipment that will be in dir6ct contact with clients. They are called
treatment rooms #1, P2, P',','5, #5, relaxation room, infrared sauna, and colonic room. The second floor
will not contain equipment of this nature.
Please let me know if there is anything else you need from me.

After speaking with b0thjon and Jeff regarding the Raphael's Joy spa that is going in on N. Range
Line, we have decided on the following course of action. (They are only repainting the exterior,
taking out dead plants and cleaning u~ the existing beds, and ~toing repair to sink-holes in the
walkway)
I will be calling Carolyn Berghuis today and letting her know that she needs to come in and apply
for an SDR and probably an ADLS Amendment for her sign. We will accept her building permit
application after we know that she has supplied Planning 6~ Zoning with the correct applications
a~l supporting documentation. We would then make sure that she received all required approvals
before We wodld issue the full C 'O.
If you have any questions, please let me know. Also, would whomever takes in those applications
and documents please let me know that they have been received, so xve will know wh~ ~ve are
okay to accept the building permit application
