I have received the required outlet permit that we talked about on the phone and it has been sent to Mike Kole for approval and processing. 2. It is also intended to identify areas where additional measures may be required to control erosion and sedimentation. All practices recommended in this report should be evaluated as to their feasibility by a qualified individual with structural practices designed by a qualified engineer. All erosion and sediment control measures shall meet the design criteria, standards, and specifications outlined in the ({Indiana Handbookfor Erosion Control in Developing Areas" or similar guidance documents. The majority of Phase I and II have been disturbed. Perimeter silt fence is installed, the pond is taking shape and storm sewer is just being installed. The pond outlet is not installed and.existing inlets or culverts have not been protected. The contractor has not made a good attempt to follow the construction sequence. Recommendations were discussed at the site. The Following Items Have Been Evaluated and Assigned a Designation of: S = Satisfactory M = Ma.rginal U = Unsatisfactory NA = Not Applicable Construction Site Management for Erosion and Sediment Control V (1) Disturbed areas have been adequately protected through seeding or other appropriate erosion and sediment control measures. Install an appropriate sediment control practice between construction areas and lower lying areas. Replace/Repair silt fence and/or straw bale barriers. Entrench silt fence (6-8 in.) and/or straw bale barriers (4 -6 in.) and stake straw bales. Remove accumulated sediment from: o sediment traps/basins, o behind silt fence/straw bales, o around storm drain inlet protection devices, o streets and gutters (Do not flush with water) X Temporary seed, fertilize, and/or mulch: . Permanent seed, fertilize, and mulch areas that are at final grade Protect storm drain inlets: o curb inlets, o drop inlets Reshape and stabilize sideslopes of : o sediment traps/basins, o detention/retention basins Install/Maintain construction entrance( s): Reshape and stabilize conveyance channels: Place greater emphasis on erosion and sediment control on building sites; contractors, subcontractors, material vendors, and others should be made aware of erosion and sediment control requirements. Appropriate perimeter sediment controls (e.g. silt fence) and stable construction entrances shall be utilized on all sites. Sediment tracked into the street shall be removed (do not flush with water) at the end of each work day. X Other Requirements: · Install inlet practice on all existing inlets or culverts. · Install outlet for sediment basin. · Temporary seed the topsoil mound. · Complete the fill on smaller sections of the pads and parking lots and apply seed, fertilizer and mulch or just mulch. The mulch must be crimped or otherwise prevented from blowing away. All disturbed areas must be protected if they are planned on being dormant for more than 15 days. · Plug the culvert on the Monon Trail as shown on the plan. · Note- Please post NOI at construction entrance as required by the Rule. The Department has completed the review of the Mass Earthwork and Building Pad Package Drawings. The City reviewed Schematic Design Drawings at the August Technical Advisory Committee (T AC) and is presently reviewing a more current set of Schematic Design Drawings that the Department received on September 15, 2004. The Department's sub-consultant is presently reviewing the drainage calculations provided at the August T AC meeting. As we discussed at our meeting on September 20, 2004, although the Department is still completing it's review of the Schematic Design Drawings and the fact that the project is still at a very early stage, the Department is ready to approve the Mass Earthwork and Building Pad Package Drawings with the understanding that the earthwork is being conducted at the Park Department's risk. Since the drawings are at the Schematic Design level, please be aware that future review of more detailed design and drainage calculations may result in comments that may require modifications to the grading to'be performed under the Mass Earthwork and Building Pad Package. Also, as the project moves into Design Development, the grading and drainage completed under this Bid Package may need to be altered out of a design necessity. This Department will forward any comments developed from the Schematic Design drainage review that might affect the Mass Earthwork and Building Pad Package when they are received. It is apparent that the Mass Earthwork and Building Pad Package agrees with the concepts presented on the Schematic Design Drawings and any comments from the drainage review are not anticipated to impact the start of Mass Earthwork. Please provide 4 sets of Mass Earth\vork and Building Pad Package drawings for approv~1. An additional 2 sets will be approved if they are provided. The construction entrance that we discussed at our meeting on September 20, 2004 will require Board of Public Works and Safety approval. Any submission to the Board requires prior approval by the Carmel Clay Plan Commission and/or the Board of Zoning Appeals (if applicable) and completion of review by the Technical Advisory Committee. The Department requires a letter from your office that formally requests that the temporary construction entrance item be placed on the next Board meeting agenda and briefly outlines the entrance and what purpose it will serve. The Department also requires an 8 12 x 11 drawing of the entrances/curb cuts. This drawing(s) will be used as an exhibit to accompany your request for Board of Works approval. These drawings should include all pertinent dimensions of the entrance, material to be used, provisions for side ditch drainage and include dimensions to the nearest drives and local streets. .Your request via email on September 21, 2004 for the construction signs would be approved at the same time as the temporary construction entrance. Please provide a drawing that details the location, size and type of signs' that you are proposing and request approval for the installation of these signs in your letter. The Departments deadline for submission to the Clerk Treasurer's Office is September 29, 2004 at 4:00. Please provide this information to the Department as soon as possible so that we may pave ample time to draft a letter of recommendation to the Board. The construction of the temporary entrance will require a Right-of-Way permit and a $2,000.00 bond. Sequencing the pipe installation is also important to divert storm water away from off site areas. The plan does not phase the construction. Areas need to be designated that will be worked and completed prior to other areas being started. B5- The sediment basin dimensions and location needs to be shown on the drawing. Provide calculations that the basin meets DNR ,Handbook requirements. If weather conditions force construction to occur other than in the pond the contractor m.ust know that the sed. basin needs to be installed prior to any mass earthwork. The existing culvert under the Monon Trail (750' north of 111 th Street) must be plugged prior to mass earthwork. B 11 & 12 Seeding specs need to include ground prep and soil amendments. Also include the 15 day restriction for disturbed areas. Temporary seeding needs to include mulch. B15- I should assume that future plans for the building site work will include SWPPP? C5. The designer is required to provide a maintenance guide for the post construction water quality practices. An individual maintenance guide or instruction is needed for the bioswales, the lagoon, turf care and forebays. I would be willing to let construction start and submit the NOI if the items in section B are addressed. Is the proposed project to be sprinklered buildings? If so a meeting will need to be scheduled to discuss the locations of the fire department connection for each building. We will be requesting exterior access to all riser rooms. 2. We are requesting the installation of a Knox box on each building. We are also requesting the Knox fire department connection plugs be install on each sprinklered building. An application for the Knox products is available on line at www.knoxbox.com 3. What are the heights of the proposed building, and will the buildings have basements? 4. Our office is requesting a set of plans showing the water distribution for this site indicating fire hydrant placements. 5. 6.
