(the °Vendor"), as City Contract dated January 19, 2011 shall amend the terms of the Agreement by adding the additional services to be provided by Vendor consistent with the Scope of Work attached hereto and incorporated herein as Exhibit "A". Furthermore, the terms of the Agreement shall be amended to include the E-verify requirement as stated in Indiana Code §22-5-1.7 et seq.,which is attached hereto and incorporated herein as Exhibit GB", as well as the required E-verify Affidavit, attached hereto and incorporated herein as Exhibit"C". The terms and conditions of the Agreement shall not otherwise be affected by this Additional Services Amendment and shall remain in full force and effect. The purpose of the additional work is to add a southbound right-turn lane on Michigan Road approaching Retail Parkway. Additional work elements required to add this to the scope are itemized as Billows: I) Perform additional topographical survey of existing features and utilities 2) Design and lay out a new southbound right-turn lane on Michigan Road 3) Perform drainage calculations and design storm sewer modifications 4) Design underdrain modifications on Michigan Road 5) Develop one plan and profile sheet 6) Develop cross sections 7) Revise the signal design 8) Revise the quantities and cost estimate 9) Revise the Erosion Control Plan and resubmit to SWCD 10) Revise and resubmit the INDOT Permit We propose to perform these additional services for a lump sum amount of$28.500.
