Hi Angie,
Just wanted to let you know I talked to Brian Cross from Civil Site. He clarified for me that they are buying about four
parking spaces from the racquet club and that it is not shared parking between the two buildings. So when I asked about
the square footage of the parking lot in relation to landscaping per the CDRL Rd. Overlay on the comments, we won't be
able to apply that provision to the other parking areas. I think we knew that, but at least they're confirming the
shared /not shared thing.

General site:
1. Please submit an overall site plan on one page. A smaller scale plan would help us to fully see the entirety of the
project. This would be helpful in your info packets, too.
2. Please show driveway connection on site plan to new Fifth Third Bank and even (possibly) onto office building to
the west.
3. Please show the square footage of the new building addition on the plans.
4. Please indicate where permeable pavers and bio- swales will be utilized.
5. Where is the dumpster located?
6. Please show what does the dumpster enclosure will look like or is planned to be. It must be similar in
construction and materials to the principle building per Ch. 19.05.02 and 23F.11.05.
7. Will there be any ROW dedication or any additional ROW dedicated? Please check with the Engineering Dept. to
see if this is required.

Thoroughfare Plan Commitment
I looked up what Fifth Third Bank did to meet the City's Thoroughfare Plan requirements. They dedicated the additional
10 feet of right -of -way required by the Thoroughfare Plan but were not required to install (or make a commitment for the
installation of) the path along Carmel Drive.
So you will only be required to dedicate additional right -of -way for your project. As I mentioned in the meeting
yesterday, we will need you to enter into a Consent to Encroach for the portion of your existing parking lot which will be
located within the new right -of -way limits. Libby Pickett in our office will be able to help you with the Consent to
Encroach process once the right -of -way is dedicated.

Dear Mr. Cross:
The following letter represents comments for this project specifically addressing the area of
alternative transportation. I have reviewed the drawings submitted for the December 19, 2012
Technical Advisory Committee meeting, and comments from the Alternative Transportation and
offer the following comments:
ALTERNATIVE TRANSPORTATION REVIEW COMMENTS
1) The City of Cannel's Subdivision Control Ordinance and Bicycle and Pedestrian Plan
requires a 10' multi -use path along Carmel Dr. Please revise the plans to reflect these
changes.
2) Please provide a pedestrian connection from the exterior multi -use path along Carmel
Dr to the buildings on the site.
3) The City of Carmel Parking Ordinance requires that all commercial developments
include bicycle parking. Bicycle parking spaces are required at a rate of five (5) bicycle
parking spaces per one hundred (100) automobile parking spaces. The City of Carmel
Parking Ordinance also requires that bicycle racks be installed within fifty (50) feet of
the entrance designated as the main entrance of the building for which the racks are
required.
4) ADA Standards for Accessible Design requires five (5) accessible parking spaces for
parking facilities that provide 101 to 150 total parking spaces. Please revise the plans
to add these spaces.
5) Please indicate ADA compliant handicap ramps at all crossings, adjacent to all ADA
parking spaces and to all of the entrances of the building(s).

We have reviewed the construction plans submitted to the Hamilton County Surveyor's Office
on November 20, 2012, for this project and have the following comments:
1. The proposed project falls in the incorporated area and MS4 jurisdiction of the City of
Carmel.
2. The proposed project DOES fall in a Carmel Wellhead Protection Zone.
3. The proposed project does partially fall in the Moffit Williamson Regulated Drain
Watershed, but all improvements and storm water discharge do not drain to a
Hamilton County Regulated Drain.
4. Please direct all stormwater questions to the City of Carmel Engineering Department.
5. The Hamilton County Surveyor's Office has no comments or concerns on this project
and no permits will be required from our office.

Dear Mr. Cross:
The City received your development plan on November 16, 2012. The project is scheduled for review at
the December 19, 2012 Technical Advisory Committee meeting. We offer the following comments:
CONSTRUCTION DRAWING REVIEW COMMENTS
1. Please provide all current easements for entire affected tracts to pertinent sheets with correct
calls for each.
2. Are any offsite easements proposed for this project?
3. Please omit Sheet Ll .0. Landscape Plan; Sheet L2.0, Landscape Plan; and Sheet SLP, Site
Lighting Plan & Photometric Plan from construction sets to be submitted for review by this
department. The Department of Engineering does not have approval authority over
landscape plans or photometric plans. Please remove the titles for the Landscape Plan and
Site Lighting Plan & Photometric Plan from the Sheet index.
4. Please provide a Drainage Summary in accordance with the Stormwater Technical Standards
Manual 102.02 xi. m. A table with the appropriate information is sufficient.
5. Please provide the 100 -year elevation of the infiltration detention pond.
6. Please indicate the bottom slope of the infiltration detention pond. A minimum I% slope is
required.
7. Please indicate a subsurface drain in the infiltration detention pond.
8. Stormwater runoff will be increased to the existing stormwater outlets in the existing Racquet
Club and Coots, Henke, and Wheeler parking lots. Please check the capacity of these outlets
and analyze the outfalls.
9. Please indicate the storm sewer system to the nearest outfall for the swale on the southeast
side of the Racquet Club property.
10. There is an existing outlet on the southwest corner of the existing parking lot that is directed
to the Fifth Third Bank property to the west. This storm sewer is not indicated on Fifth
Third's construction plans. Please contact Aaron Lane, the project engineer at GPD Group,
614- 859 -1611, to determine the condition of this pipe.
11. Please verify the capacity of the existing pipe, west of the Racquet Club's property, which
the detention pond will outlet. Please check the hydraulic grade line of this pipe and
downstream outfall at Medical Drive.
12. The existing Racquet Club roof drainage, at the south side of the building, needs to be
intercepted and directed to the detention pond. There are existing drainage issues on the
south side of the Racquet Club building which need to be corrected with this project. The
opening inthe pipe in this area needs to closed back up, and the roof downspouts need to be
enlarged to accommodate the full amount of ninoff from the building in this area.
13. All construction on the south side of the property, near the proposed detention facility, should
be careful due to the existing retaining walls along this property line.
14. Portions of new impervious surfaces are not indicated to be detained with this project. New
impervious surfaces shall be detained.

15. Will protection/guard rail be provided along the north side of the detention facility?
16. Please provide more spot shots along the drive leading from the existing Racquet Club
parking to the proposed parking lot.
17. Please include the City's Curbing Policy in the plan set.
18. The Thoroughfare Plan indicates a 50 -foot half right -of -way for Carmel Drive. Please
indicate this on the plans. A Consent to Encroach will be required for the existing Coots,
Henke, and Wheeler parking lot.
GENERAL INFORMATION
19. These comments represent the Department of Engineering's first review of the development
plan for this project,
20. We request that all responses to our continents be provided in writing and be accompanied by
a drawing reflecting the requested revisions. Failure to provide written responses may result
in the delay of the review process.

22. We have engaged Crossroad Engineers, PC to review all drainage plans and drainage
calculations submitted to this office for review. if you have not already done so, please
provide a set of drainage plans and calculations to their office for review. We will share
Crossroad's comments as they are received.
23. Final drawings will not be approved for construction until:
a. All Engineering Department and Utility Department and Hamilton County Surveyor
issues have been resolved.
b. All bonds and perfonnance guarantees are posted.
c. All Board of Public Works and Safety approvals and any other governing agency
approvals (if required) are obtained.
d. All off -site easements necessary to install utilities to serve the development are secured.
e. SWPPP is approved.
f. All fees are paid.
24. The Department reserves the right to provide additional comments based upon subsequent
reviews.
25. An approved Storm Water Management Permit is required prior to commencing any earth
disturbing activity. Please contact Mr. John Thomas regarding storm water quality
requirements.
26. An approved right -of -way permit is required prior to commencing any work in the public
right -of -way and for construction equipment access from the City's right -of -way.
27. If it will be necessary to relocate existing utilities, the costs for such relocation shall be borne
solely by the developer. Any utility poles requiring relocation shall be relocated to within
one foot of the outside edge of the proposed right -of -way.
28. The Department requires that the construction drawings be developed in accordance with the
City of Carmel digital submission standards and that all required submittals for primary plat,
secondary plat, and construction drawings be made. The digital files must be submitted to
the Department of Engineering prior to the approval of the construction plans. Please contact
the City G1S Department for the requirements.
29. Jurisdictions:
a. The project site is located within current City of Carmel Corporate Limits.
b. Perimeter Street and Right -of -Way — City of Carmel (Carmel Drive)
c. Water — City of Carmel Utilities
d. Sanitary Sewers — City of Carmel Utilities

30. Drawings submitted for approval:
a. The design engineer must certify all drawings submitted for final approval.
b. This office will require 9 sets of drawings for approval after all issues have been
resolved. The drawings will be stamped as approved and signed by the City Engineer
and by Carmel Utilities. The Owner will receive 3 sets, one of which must be maintained
on the construction site at all times. If this project is subject to review and approval by
the Hamilton County Surveyor's Office, a total of 11 sets will be required for final
approval.
31. Carmel Utilities will provide separate reviews of this project for water issues. Please assure
that copies of all drawings are sent to:

BOARD OF PUBLIC WORKS AND SAFETY
34. A schedule for Board of Public Works and Safety meeting dates and agenda deadlines will be
sent electronically for your use upon request. Please use the Engineering Department
deadlines for submissions to the Board.
35. Any submission to the Board requires prior approval by the Carmel Clay Plan
Commission and /or the Board of Zoning Appeals (if applicable) and completion of
review by the Technical Advisory Committee. All written requests to be placed on the
Board's agenda must include the appropriate Docket Number and the date (or dates) of
approval by the Plan Commission and /or the Board of Zoning Appeals (if applicable).
36. Water Availability and Sanitary Sewer approval from the Board will be required. This is an
EDU approval based upon the proposed use of the site. Reference Items #33 to #35 below
for additional details /explanations. Please note that if an entryway or other irrigation
system is planned for this development, additional Water Availability Approval from
the Board will be required and additional Water Connection Fees will be assessed based
upon the size and usage of the system as determined by the Director of Carmel Utilities.
37. Commercial Curb Cut Approval. Please provide 8'/ x 11 exhibits with the request for
approval. Provide all pertinent information including lane widths, overall width, radii, lane
markings, location of opposing drives or streets, relationship to the location of previous curb
cut, etc.
38. Temporary Construction Entrance Approval. A construction entrance is not shown on the
development plan, but will be required on the construction plans when this project gets to
that stage.
39. The installation of any permanent, privately owned and /or maintained improvement (signs,
decorative street signs. walls, streetlights, etc.) within dedicated right of way or dedicated
easements requires the execution of a Consent to Encroach Agreement between the Owner
and the City of Carmel. Such agreements are executed by the Board of Public Works and
Safety. The City Engineer may approve irrigation system agreements.
40. Secondary Plat approval if applicable. All performance guarantees must be posted prior to
submission of secondary plats for Board of Public Works and Safety approval.

41. Dedication of right -of -way if not platted. This is based upon the City of Carmel 20 -Year
Thoroughfare Plan requirements. Dedication documents are available upon request. Please
be advised that all Right-of-Way Dedications must be accompanied by a Sales Disclosure
Agreement completed by the owner for the property being dedicated to the City. The
dedication document cannot be recorded without a completed Sales Disclosure. The form is
available upon request.
42.' Any open pavement cuts of City right -of -way will require Carmel Board of Public Works
and Safety approval.
BONDING REOUIREMENTS
43. Please contact Mr. Dave Barnes to review performance guarantee requirements. Please
contact Mr. John Duffy to review water and sanitary sewer bonding requirements.
44. The amount of the Performance Guarantee is based upon a certified Engineer's Estimate for
100% of the cost of labor and materials to construct the individual improvements, to be
provided by the design engineer. Please provide detailed Engineer's Estimates for each
improvement including quantities, unit costs, pipe sizes, and materials, etc.
45. Upon completion and release of individual Performance Guarantees, a three -year
Maintenance Guarantee will be required (see Street Sign comments above). The
Maintenance Guarantee amount is based upon 15% of the Performance amount for Streets
and Curbs and 10% of the Performance amount for all other improvements.
46. Performance Guarantees may be Performance or Subdivision Bonds or Irrevocable Letters of
Credit.
47. Please reference the available enclosures for more detailed explanation of our procedures.
RIGHT OF WAY PERMIT AND BONDING
48. Any work in the dedicated right -of -way will require an approved Right -of -Way Permit and a
License & Permit Bond.
49. The bond amount is determined by our Right -of -Way Manager. However, if the work is
included in the scope of work of a required and posted Performance Guarantee, the
Performance Guarantee may be used to satisfy the bond requirements of the Right -of -Way
Permit.
50. Please contact our Right -of -Way Manager, Fred Glaser, to arrange right -of -way permitting
and bonding.
AVAILABILITY AND CONNECTION FEES
51. We defer to Carmel Utilities regarding this issue.
52. if an entryway or overall site irrigation system is planned for this development,
additional Water Connection Fees will be assessed based upon the size and usage of the
system and upon the recommendations of the Director of Carmel Utilities.
53. These fees are required to be paid prior to final approval of construction plans by
Engineering and prior to issuance of building permits by Building Codes Services. Please
confirm these fees and calculations with Carmel Utilities.
