The underground utilities depicted on the attached plat of survey have been located per field observations only. No
warranty, either expressed or implied, is made to the accuracy and /or completeness of information presented on
underground utilities, or as to its fitness for any particular purpose or use. In no event will Roger Ward Engineering, Inc.,
its employees, agents, and /or assigns be liable for any damages arising out of the furnishing and /or use of such
information. The path of the utility lines on said plat of survey should be considered approximate until they are either
relocated, by calling the Indiana Underground Plant Protection Agency at 1- 800 - 382 -5544 or until they are excavated to
verify the location and path of the utility lines.
The above described real estate does lie within a Zone X Unshaded Floodplain Area, an area determined to be outside
the 500 -year flood plain, as shown on the Flood Insurance Rate Maps Community Number 18057CO205 F, prepared by
the Federal Emergency Management Agency, dated February 19, 2003. Attention is called to the fact that there are
substantial uncertainties associated with said map and with information taken therefrom. Flood Plain information is
provided solely based upon a review of the Flood Insurance Rate Maps, and shall not be construed as a confirmation or
denial of flooding potential.
The owner of the subject property per records obtained from the Hamilton County Recorder's Office is Fitzgerald per

The thpurpose of this survey was to perform a topographic investigation of a portion of the property located at 1616 West
116 Street. Field work for this project was performed during the months of December & January, 2004 & 2005. A
retracem ent survey of the subject property executed in accordance with the requirements of the Indiana Survey Standards
as defined in Title 865, Article 1, Rule 12 of the Indiana Administrative Code was not performed as a part of this
topographic investigation.
BENCHMARK NOTE
Elevation were established based upon control taken from an ALTA /ACSM survey by Mid - States engineering, job number
122 -0048 dated 2/13/2004.

Seedbed Preparation
Apply limo to raise the pH to the level needed for
seeded. Apply 23 pounds of 12 -12 -12 analy -ta fertilizer ( por
1000 sq. ft. (approximately 1000 pounds per acre) or fertilize according to
test. Application of 150 lbs. of ammonium nitrate on areas low In organic
matter and fertility will greatly enhance vegetative growth.
Work the fertilizer and line Into the soil to a depth of 2 -3 Inches
with a harrow, disk or rake operated across the slope as much as po3ble.
Seeding
Select a seed mixture based on projected use of the area (Figure 5-2),
while considering best ceedIng dates. See Figure 5-3 this sheet. If toler-
ances are a problem, such as salt tolerance of seedinga adjacent to streets
and highways„ we Figure 5-4 this sheet before final "action.
Mulch Rate
Mulch is to be applied at 2.000 to 3,000 pounds per acne in arecm not
covered by erosion control blanketing. mulch must be anchored using a mulch
anchoring toad or farm disk with dull, serrated, straight set blados, or bulldozer
cleats driven up and down slope.

Tree Protection For Construction Sites
1 Protective Maintenance: The contractor shall provide maintenance
and care for existing trees regained to be protected on site and adjacent to any project
or constrnction site during construction activities and to ensure that existing trees survive
and are not damaged
2 SoIl Compaction Prevention
To prevent soil compaction, designated mutes for equipment and foot traffic by
work crews shall be determined prior to commencing construction
The contractor shall inform all construction crew members on the site of access
mutes and will ensure that only these routes are used.
To prevent tree root smothering, no soil stockpiles, supplies, equipment, or any
other material shall be placed or stored within a tree dripline or within 15 feet of the
tree hunk for column-shaped tees, whichever distance is greater.
-Soil around a tree dripline may be required to be aerated during and after
construction activities. This is necessary even when compaction results from henry
foot traffic.
3 Root Protection
-Tree mots shall not be cut unless cutting is unavoidable.
-When root cutting is unavoidable, a clean, sharp cut shall be made to avoid
shredding or smashing. Root cuts should be made back 6T]a terfil toot
- Whenever possible, tree roots should be cut between late fall and bud opening, when
root energy supplies are high and conditions are least favorable for disease causing
agents-
4 Tree Fencing
- Fencing material shall encircle any tree whose outer dripline edge is within 20 feet of
any construction activities.
- Fencing material shall be a bright, contrasting color, durable, and at least 4 feet high.
-Fence posts shall be comparable to metal T -posts or heavier posts and placed at
least 2 feet below ground
- Fencing material shall be placed at the dripline or at least 15 feet from any tree tnmk,
whichever distance is greater, and maintained in an upright position throughout the
duration of construction activities.
-Heavy objects, such as wood pallets or metal railings, shall not lean against or come
into contact with any tree trunk
5 Grade Changes: Grade changes, such as removing topsoil or adding fill material, shall not
occur within the dripline of any existing tree. If necessary as part of project or site
development, retaining walls and tree wells to maintain the existing grade within the dripline
of any tree may be acceptable when constructed prior to site grading changes near the tree.
