Between Stivers Staffing Services and Carmel Redevelopment Commission
1. STATEMENT OF WORK
This is an agreement made as of March 31, 2008, between Stivers Staffing Services
(hereafter called Stivers) and Carmel Redevelopment Commission to provide payrolling
services for people to work under Carmel Redevelopment Commission direction, control,
and supervision.
2. PAYROLLEES
Stivers is the employer of record for all payrollees that you have requested us to put on
our payroll. Stivers will pay the weekly wages and will make all appropriate tax, social
security, Medicare and other withholding deductions and will make payments and will
provide worker's compensation insurance coverage and make all appropriate
unemployment tax payments. We will charge you a 30% markup added to their hourly or
weekly pay rate for our services. Some additional services that you may require, we will
handle for no additional cost to Carmel Redevelopment Commission.
3. INSURANCE AND LIABILITY
Stivers maintains Worker's Compensation and General Liability insurance coverage
relative to claims under Worker's Compensation Acts and from any claims for bodily
injury, death or property damages for which Stivers may become liable under the terms
of this agreement.
4. HIRE POLICY
Under this agreement, any of the payrolled people may at any time be converted to your
own permanent payroll for no additional charge.
5. BILLING AND PAYMENT
Stivers payrollees will submit weekly timecards with supervisor approval to us in at-
timely manner. Stivers will then submit weekly invoices to Carmel Redevelopment
Commission for the hours /time worked by each payrollee. Terms are Net 30. Overtime
will be paid for hours worked by non- exempt payrollees in excess of forty (40) hours per ,
week. Overtime will be billed at 1.5 times the standard bill rate.
6. EQUAL EMPLOYMENT OPPORTUNITY
Stivers and Carmel Redevelopment Commission agree to apply uniform and equitable
standards of employment opportunity to assure that appropriate use is made of the
abilities of the payrollees regardless of race, creed, color, religion, national origin,
ancestry, nationality, marital status, handicap, status as a disabled or Vietnam veteran,
sex or age. Both Stivers and Carmel Redevelopment Commission shall be responsible
for their own violations and the violations of their respective employees in this regard.
7. ACCEPTANCE
Payrollees are provided only in accordance with the terms contained herein and shall be
deemed to have been accepted in accordance with said terms.

8. AMENDMENT
This agreement may be amended only by a written agreement between Stivers and
Carmel Redevelopment Commission that expressly amends, terminates, or supercedes
this Agreement.
9. LOCAL AND STATE LAWS
Stivers is required to follow applicable local and state laws.
