As required by referenced commitments, enclosed are the latest revisions of the Spill
Prevention Control and Countermeasures (.SPCC) Plans for North Indianapolis and Carmel
Sand. The Plan for North Indianapolis covers Mueller Property South. The Plan for Carmel
Sand covers Mueller Property North. The enclosures should be inserted in your copy of the
plans per the instruction included.

Replace Pages 5 and 7.
Add Page 32 to Appendix C after the blank page entitled "Detailed Discharge Report
Form".
Add the page entitled "SPCC Plan Training" and the eight pages that follow to Appendix
D after the page entitled "Record of Discharge Prevention Briefings". Training
information for 2007 and earlier years may be removed from the book.
in the east end of the Upper Sump, in Blue Woods Creek on the east side of the Carmel Concrete crossing and
where the lakes west of Gray Road overflow into Blue Woods Creek. An absorbent material purchased from
Chem Search was also used. Brian Smith was the IDEM contact. Incident No. 2007-03-266 was assigned to this
spill. Duke's Earth Service was hired to clean up the spill. They skimmed 475 gallons of oil and water from the
• Upper Sump. Also contacted were Jerry Liston of the Hamilton County Surveyor's Office(Blue Woods Creek is a
legal drain under their jurisdiction)and the City of Carmel Water Utility.
A spill of approximately 45 gallons of crusher oil was spilled at the wash plant on September 18, 2007 at
approximately 9:30 PM. A rupture in an oil line caused the spill. Booms were placed in the sump adjacent to the
wash plant and around the pumps in the west end of the Upper Sump. All pumps in the Upper Sump were shut off
after the spill occurred.. Brian Smith was the IDEM contact. Incident No. 2007-09-119 was assigned. Martin
Marietta personnel performed the recovery of the spill.
On February 14, 2011 F.E. Harding notified plant management that a trailer used by Martin Marietta's recycle
plant operation had been overturned. The trailer had a fuel tank on it. Marks on the side of the trailer indicate
that it was hit. When and how this occurred is unknown. The amount of fuel estimated to have been spilled was
five gallons, below any reporting limits. Booms and pad were used to absorb the spilled fuel. This area is
within the City of Carmel wellhead'protection. As a result, Paul Pace of the Utility was notified because the •
utility stated previously that they would like to know of any spill within their wellhead protection area.
On March 8, 2012, during repairs on the cone crusher on the pit floor, the lubricating oil pump was started while
the oil line was disconnected. It was an oversight with personnel involved not knowing that the line was
disconnected. Booms and pads were used to absorb as much of the spill as possible. The spilled oil was
confined to a low area on the pit floor., Fines were used to create a dam around the spill to prevent it from
spreading. IDEM's Office of Emergency Response (317-233-7745) was contacted. A voice mail from Patrick
Colcord (317-691-6667)was received and the call returned. He said no further action was necessary. Incident
No. 27575 was assigned. Investigations on March 9, 2012 indicated that the absorbent materials had collected
the spill with no evidence of any reaming oil left in the low area. Likely the spill was much less than 50 gallons
• based on how easily it was absorbed. Paul Pace of the City of Carmel Water Utility (317-733-2858) was
contacted on March 9, 2012. No contact was made but a voice mail was left. He returned the call and did not
express concerns due to the size of the spill but he did express thanks for the call.

40 CFR 112.5(a), (b) and (c) requires the SPCC Plan to be amended whenever there is a change in facility
• design, construction, operation, or maintenance, which materially affects its potential to discharge to navigable
waters of the United States or adjoining shorelines. In addition, the owner or operator must complete a review
and evaluation of the SPCC plant at least once every five (5) years. Amendments to the Plan shall be made
within six (6) months of the change or review, with technical amendments certified by a Professional Engineer
(PE).'Administrative updates such as name or phone number changes do not require PE certification.

Note:
The owner or operator must complete a review and evaluation of the SPCC plant at least once every five (5)
• years. The Plan must be amended within six (6) months of the review to include more effective control and
prevention technology, if such technology has been field-proven at the time of the review and will significantly
reduce the likelihood of a discharge.

DETAILED DISCHARGE REPORT FORM
Reporter's Name and Date: Max Williams
Location of Discharge: Pit floor adjacent to crusher.
Date and Time Discharge Occurred: Approximately 3:30 PM on March 8, 2012. Received call
from Phil Rowen at 4:18 PM.
Material and Amount Discharged: Estimated 50 gallons of crusher lubricating oil.
Source of the Release: Disconnected oil line.

of Emergency Response (317-233-7745) on March 8, 2012 at 5:12 PM. Received return voice mail
from Patrick Colcord (317-691-6667) at 5:59 PM. Returned call after reviewing voice mail at 6:38
PM. Described incident. IDEM said no further action was necessary. Incident No. 27575 was
assigned.
Paul Pace of the City of Carmel Water Utility (317-733-2858) was called at 12:31 PM on March 9,
2012. No contact was made but a voice mail was left. He returned the call at 1:29 PM. He did not
express concerns due to the size of the spill but did expressed thanks for the
call.
Corrective Actions Implemented to Prevent Recurrence of Discharge: Will pay closer attention in
the future to look for instances where spills could occur due to similar circumstances.

The following pages are the agenda and attendance lists from annual training
classes for the employees of North Indianapolis, Noblesville Stone, Kentucky
Avenue Mine, Kokomo Stone and Kokomo Sand. The classes were held at
