Dear Mr. Olds:
Golars, LLC d/b/a Golars Environmental and Remediation Services (Golars) is pleased to
submit this proposal to conduct Pre-Demolition Environmental Assessment Services at the
above referenced property (Site). The proposed services will include a Hazardous Building
Materials (HBM) Survey and a Limited Surface Investigation.
The Site is approximately 6.06 acres improved with one approximate 50,000-square foot
commercial (former industrial) building located on the central-eastern portion of the Site, and one
garage building located on the west portion of the Site. The Site is adjoined on all sides by
commercial properties.
PROJECT UNDERSTANDING
Golars understands that the Carmel Redevelopment Commission (CRC) intends to redevelop
the Site. Existing site improvements will be razed, followed by redevelopment for beneficial
reuse. Therefore, the purpose for conducting the Pre-Demolition Environmental Assessment
Services is: (1) evaluate the existing buildings for HBMs that might affect demolition, and; (2) to
evaluate the building's interior concrete surface in targeted locations for the presence of
polychlorinated biphenyls (PCBs).
Golars conducted a Phase I ESA at the Site and prepared a report of Phase I ESA dated
December 21, 2012. The Phase I ESA identified several Recognized Environmental Conditions
(RECs) associated with the Site. The identified RECs are described below in the Limited
Surface Investigation Section.

PRE-DEMOLITION ENVIRONMENTAL ASSESSMENT SERVICES
HAZARDOUS BUILDING MATERIALS SURVEY
In anticipation of the CRC's intent to prepare a request for bids for demolition of Site buildings
and improvements, Golars will conduct a survey for HBMs:
• Polychlorinated Biphenyl (PCB)-containing Equipment
• Mercury
• Chlorofluorocarbon (CFC)-containing Equipment
• Tritium Exit Signs
• Suspect Asbestos-Containing Material (AGMs)
• Lead-Based Paint(LBP)
The HBM survey will consist of a site reconnaissance and an inventory of suspect HBMs in the
site buildings.
The ACM Survey will consist of the collection of suspect ACMs, including roof material. The
ACM survey will be an AHERA level survey. A separate report of ACM Survey will present
findings and conclusions such to identify ACMs that require abatement prior to demolition and
will identify ACMs that can be disposed of with the other non-ACM building materials.
The LBP Survey will evaluate painted surfaces for lead content. A separate Report of LBP
Survey will present findings and conclusions such to identify LBP that requires abatement prior
to demolition and will identify LBP that can be disposed of with the other non-LBP building
materials.
A report of HBM Survey will identify HBMs that require action to be taken prior to demolition and
HBMs that can be managed during demolition.
In addition, Golars will prepare a separate letter which will summarize the findings and
recommendations to address the proper management of HBMs. Golars will also prepare a
separate document formatted similar to a demolition bid specification for HBM management.
LIMITED SURFACE INVESTIGATION
The purpose of the limited surface investigation is to collect surface concrete samples from
inside the main building in areas beneath the mini-transformers identified during Golars' Phase I
ESA, A Site Map showing proposed sampling locations is attached to this Proposal. A brief
summary of the REC identified in the Phase I ESA are described below:
• REC 8: PCB-containing mini-transformers were identified hanging from the ceiling in
various locations throughout the warehouse portion of the main building (labeled CAP1,
CAP2, CAPS, and CAP4, respectively). Seven locations or suspect former locations
were identified during the Phase I ESA site reconnaissance.
The scope of work for the Limited Surface Investigation only includes investigation of REC 8.
Other RECs identified during the Phase I ESA will be addressed in a separate scope of work.
The specific scope of the limited surface investigation is described below:

1. A site health & safety plan will be prepared for Golars personnel.
2. To address REC 8, Golars will conduct the investigation scope of work summarized below.
The purpose of the concrete chip sampling is to evaluate for the presence of PCBs adhered
to the concrete floor's porous surface in the immediate vicinity beneath each of the seven
(7) suspect PCB-containing equipment locations identified during the Phase I ESA.
• Four concrete chip samples will be collected from each of the seven (7) suspect PCB-
containing equipment locations, thus a total of 28 concrete chip samples. Each concrete
chip sample will be retrieved from an approximate 100 square centimeter area, to a
depth of approximately one centimeter below the top of concrete surface.
• Each concrete chip sample will be submitted to the laboratory for analysis of PCBs.
3. All investigation derived waste generated from work described herein will be staged on-site
for future handling by the client. Investigation derived waste from the concrete chip
sampling activities will be containerized and labeled with a suspect PCBs label and staged
at the Site pending the results of laboratory analysis.
4. A letter report of limited surface investigation findings will be prepared and one (1) copy
provided. The Limited Surface Investigation Report will include a description of the scope of
work, field activities, analytical results, findings, and conclusions. The findings of concrete
chip sampling will also be included within the Report of HBM Survey. A separate
recommendations letter will be prepared if specific recommendations are identified.
The Pre-Demolition Environmental Assessment Services may result in development of
information that may place an obligation upon the site owner or operator to provide reporting to
a regulatory agency or other third party. Golars will not provide reporting to regulatory agencies
or other third parties unless Client expressly requests such reporting to be performed.
SCHEDULE
Golars proposes to deliver the report in 25 business days after we receive written authorization
to proceed. Golars will need to receive a signed copy of this proposal to proceed with the scope
of work outlined herein. Completion of the project may be affected by access to the property,
the availability of information, and other factors.
SPECIAL INSTRUCTIONS
Upon project start-up, Golars will contact you or your designated representative regarding this
information and project scheduling. In addition, the client must provide documentation to
determine existing subsurface utilities or structures in order to allow proper placement of the
boring locations. In addition, Golars will engage a private utility marking service to survey the
areas prior to installation of soil borings.
Arrangements for access, including notification of tenants, will be the responsibility of the client.
Client shall provide clearance for all drilling and boring locations. Golars is not responsible for
subsurface utilities or other appurtenances not identified by the client or utility alert services or
private utility locators notified prior to the start of drilling activities.

THIRD PARTY RELIANCE
Third party reliance letters may be issued upon request and upon the payment of the then-
current fee for such letters. All third parties relying on Golars reports, by such reliance, agree to
be bound by this proposal and Golars General Conditions. No reliance by any party is permitted
without such agreement, regardless of the content of the reliance letter itself.
FEES
It is proposed that the fee for performance of the outlined scope of services be determined on a
Time & Materials basis. Based on the scope of services outlined above, it is estimated that the
total fee will be $21,460. The table below presents a breakdown of the total estimated fee.

If a lift is necessary to access sample locations, it will be charges extra at cost plus 10%, plus
labor hours. The proposed cost is based on a standard laboratory turnaround of 7-10 working
days. Optional expedited results are available upon request at an additional charge of 2.0 times
the laboratory costs. Additional work required beyond the scope of services included in this
proposal, or as caused by factors beyond Golars' control, will be invoiced on a time and
expense basis. Additional work will not be performed without prior authorization.
AUTHORIZATION
To authorize our services, please sign and complete the attached proposal authorization and
instructions for payment form, and return one copy of the authorized proposal to our office. We
will proceed with the work upon receipt of proposal authorization. Golars will perform the work
in accordance with the attached General Conditions, which are incorporated into and made a
part of this proposal.
