Executive Summary
Carmel-based developer Mainstreet Property Group will provide plans for a new 104-bed Short
Stay Rehab and Assisted Living Facility. The facility will provide post-hospital rehab services
and long-term care for Carmel area residents requiring these services.
The current design brings new life to the long-term care industry by working to shed the old
common methodology for a facility of this type, paving a new way of thinking. In turn, it will
provide an environment with a comfortable hospitality feel and will have activity-packed social
destinations, including spaces such as restaurant dining, game and activity rooms, a home
theater, and a café. In addition to the social spaces, there will be the option to retreat into a care-
based environment where the resident can receive a massage experience and/or rest and relax in
a private room setting.
The exterior facade design will move away from the traditional 1-story layout of these types of
facilities and instead provide a modern 2-story contemporary look through the use of materials
such as brick, variable sizes of fiber cement siding boards, aluminum storefront, and a water-
resistant “rainshadow” panel system.
The project site is 7.98+/- acres located east of North Pennsylvania Avenue, just north of the City
Center Drive. The site has frontage along North Pennsylvania Avenue and will be accessed from
an existing private drive. New public improvements include a 10 foot wide asphalt trail and
street trees in accordance with the current thoroughfare plan.
Mainstreet Property Group requests the Plan Commission’s approval of its DP/ADLS application
and support for the project moving forward.
