Fire Department - 2013 Appropriation#43-501.00: P.O. #24466 Contract Not To Erceed$5,900.00 ADDITIONAL SERVICES AMENDMENT TO AGREEMENT FOR PURCHASE OF GOODS AND SERVICES �geRO`4 41.6 act THIS AMENDMENT TO THE GOODS AND SERVICES AGREEMENT ("Agreement") entered into by and between the City of Carmel and CE Solutions, Inc., (the 'Vendor"), as City Contract number 04.07.10.02, dated April 7, 2010 shall amend the terms of the Agreement by adding the additional services to be provided by Vendor consistent with the Scope of Work attached hereto and incorporated herein as Exhibit"A". Furthermore, the terms of the Agreement shall be amended to include the E-verify requirement as stated in Indiana Code § 22-5-1.7 et seq., which is attached hereto and incorporated herein as Exhibit "B", as well as the required E-verify Affidavit, attached hereto and incorporated herein as Exhibit "C". Also, pursuant to I.C. § 5-22-16.5, Vendor shall certify that in signing this document, it does not engage in investment activities within the Country of Iran. The terms and conditions of the Agreement shall not otherwise be affected by this Additional Services Amendment and shall remain in full force and effect. (CES)respectfully submits the following Combined Professional Services and Fee Proposal for structural engineering services on the captioned project. Our proposal is based on the following information: n ii • There is continued concern/perception that the concrete slab-on-grade in the kitchen, day room and bunk room is settling. Gaps have been observed between base boards, " bottom of cabinetry, door frames, etc. and the top of the floor slab as well as the top of wbunk partitions and the ceiling. CES intends to utilize the land surveying services of Woolpert. Inc. to obtain relative top-of-floor slab elevations in the described areas to establish approximate limits and magnitude of perceived displacement. These elevations will be correlated with those taken during our 2009 structural condition assessment. • Review original /record construction documents(drawings and specifications)and other pertinent construction records for the existing building and remodel (should they become available). • CES will prepare a structural investigation report describing our findings and recommendations. SCHEDULE CES'workload is such that we can begin our work upon notice to proceed. FEE CES intends to provide the above professional services on a firm fixed fee basis as follows: Structural Investigation and Report $5,900 (includes $2,500 for land surveying services by Woolpert. Inc.) Reimbursable expenses are identified on the attached CES Fee Schedule and are in addition to our fee. The above fees are based upon our services being provided in 2013. Should the project be delayed, our fees are subject to modification based upon actual market fluctuations. ADDITIONAL SERVICES , The following additional services are not included in our fee, but will be provided upon request in accordance with the attached CES fee schedule and an agreed upon fee: • Preparation of an opinion of probable structural construction cost for recommended repairs, augmentation or strengthening of structural deficiencies observed and noted in the existing building. • Significant post-report clarification, meetings, response to questions. etc. • Structural Design and Construction Document preparation for recommended repairs. augmentation or strengthening of any structural deficiencies in the existing building. • Structural analysis of the existing building or the evaluation of the building's original structural design. Combined Professional Services and Fee Proposal Fire Station No. 44, Carmel, IN btdil Cannel Fire Department Follow-up Floor Investigation April 28, 2013 Page 3 • Destructive (invasive) structural investigation to uncover suspected/ potential hidden structural deficiencies. • State Application for Construction Design Release, filing fee and submission of necessary documents to the local and State building authorities. • Printing of Construction Documents(plans and specifications)for bidding purposes. • Bidding Phase services. • Structural services and consultation during the Structural Construction Administration Phase for recommended repairs, augmentation or strengthening of any structural deficiencies in the existing building designed by CES. • Construction change orders necessitated by revisions in the Structural Contract Documents beyond CES control, or by redesign efforts made necessary because of unforeseen existing conditions. • Preparation of Record (As-Built)drawings showing significant changes in the structural work made during construction based on marked-up prints, drawings. and other data furnished by the Contractor. EXCLUSIONS The following services are not currently offered by CES and are excluded from the Scope of Basic and Additional Services: • Engineering services related to construction means and methods (erection sequencing, temporary bracing, job site safety, etc.). • Determination of the fire rating required for the structural systems and components. • Geotechnical engineering report with recommendations. • Structural instrumentation, monitoring or testing. • Phase 1 and 2 Site Assessments, Asbestos and Lead-Based Paint Surveys. • Radon, mold, mildew, and moisture infiltration investigation, testing, and mitigation services. • Design and detailing of foundation drainage systems. • Development of waterproofing details and technical specifications. • Civil/Site Development services.
