RE: Worthington Estates, Sec 4, Lot 201 (#04120021 SP) - Replat Application
Dear Mr. Barnes'
This letter is in response to your Replat Application for Worthington Estates, Sec 4, Lot 201.

Please add docket no. 04120021 SP to sheet 1.
Show/label road right of way on Kirkgate Ct., adjacent to the lot.
Please add a location map to sheet 1.
Please change the lot number to 20 lA.
Remove text located in lot 201 that states: part of Block A.
Please show the source of title of ownership on sheet 1.
Please show the thin, solid line as a dashed line, on the east side of the lot where it delineates
the 10-ft DE/UE.
Please submit a revised Secondary Plat with the above-mentioned changes. Once these changes have
been made and comments addressed, please produce a Mylar copy for signature by this Department.
Two paper copies of the plat must be filed with the Mylar prior to recording, and three paper copies and a
Mylar of the recorded plat must be submitted following recording. Our approval of the secondary plat
(signature) will be forthcoming upon revision of the plans according to the items listed above and with
the approval of the Primary Plat Amendment by the Plan Commission.
Also, the staff has been advised that we cannot present a Secondary Plat to the Director for signature
from this point forward until Terry Krueskamp in Information Systems has received an e-mail with an
electronic file attached (MicroStation .dgn is preferred, or AutoCad .dwg of the subdivision) and it (the
e-mail) has been copied to Matt Griffin and myselfi Their e-mail addresses are:
