We have reviewed the construction plans submitted to the Hamilton County Surveyor's
Office on. June 16, 2004 for this project and have the following comments:
NOTE: Existing drainage problems exist in the J.W. Brendle Regulated Drain
Watershed and it has been approved by the Hamilton County Drainage Board
for n complete watershed study to be conducted. No additional development in
this watershed will be approved until this study is complete and corrective
action is taken to address the drainage problems.
1. The proposed project falls in the jurisdiction of the City of Carmel
2. The proposed project DOES NOT fall in a Carmel Wellhead Protection Zone.
3. This project will be a Hamilton County Regulated Drain Subdivision.

Thc proposed project falls in the J.W. Brendle Regulated Drain Watershed. This
watershed has an allowable release rate of 0.24 cfs per acre.

Please submit the following procedural items to this office, petition for regulated
subdivision, outlet permit, 4 crossing permits (2 water, 2 sanitary sewer- one
form) engineer's estimate, and all associated bonds for 100% of construction cost.

Please show and cross.section an emergency spillway and emergency routing at
thc 100 yc~ elevation. The emergency routing must trove a positive fall route
away from thc pond to an adequate outlet.

Please consider using castings that are designed for bi-directional flow in the
"sag" of the streets, The acceptable castings am Neenah 3501-L2 or East Jordan
749~-M4. The N¢¢nah (or equivalent) 3501 TR or TL should be used when a
single direction of flow enters the inlet.

All casting should be labeled "Dump No Waste- Drains to Waterway," which are
readily available now from local casting distributors. Please add a note to thc
plans that will make the contractor away of this requirement.

Please remove note "All curb inlet castings to be Neenah R-3 $01-N" on C600.
The casting need to be Neenah R-3501-(TR, TL, or L2) depending on the inlet
flow direction.

Dear Mr. Olmstead:
This letter is in response to your Secondary Plat Application for Lakeside Park, Sec. 3. Additional
comments were be made by members at the TAC Meeting on June 16. Please complete the following:
1. NOTE: TAC members should receive plans 15 day prior to the TAC meeting for adequate
review. If not, the petitioner runs the risk of being tabled to the following month's TAC
meeting.
2. Please place the subdivision title heading on each sheet.
3. Please provide an area location map on sheet 1.
4. Please add docket no. 04060005 SP to the plat.
5. Please add surveyor's signature to the plat.
Construction Plans:
6. Correct the text on the signature sheet (first paragraph on the right side): change the text to
read Section 3, not Section 2.
7. Provide a page describing the water, sanitary sewer, and storm drainage systems.
8. Provide a copy of covenants, if any.
9. On construction plan: show/provide sidewalk ramps on both sides of the street between lots
90-91 for access to Common Area across the street.
10. Please provide a larger, engineered landscape plan of Section 3.
Please submit a revised Secondary Plat with the above-mentioned changes. Once these changes have
been made and comments addressed, please produce a Mylar copy for signature by this Department.
Two paper copies of the plat (with owners' signature) must be filed with the Mylar prior to recording,
and three paper copies and a Mylar of the recorded plat must be submitted following recording. Our
approval of the secondary plat (signature) will be forthcoming upon revision of the plans according to the
items listed above.

We have reviewed the construction plans submitted for the June 6 Technical Advisory Committee meeting.
We offer the following conunents:

· Temporary Construction Entrance (if applicable and if located at a location other than planned
permanent entrances)
· Please be advised that any open cuts of dedicated streets will require separate Board of
Public Works attd Safety approval.
· Any permanent improvement (including irrigation systems) to be installed within dedicated
right of way or dedicated easements. This approval would require a Consent to Encroach
Agreement between the Owner and the City of Carmel.

I am also enclosing a schedule for Board of Public Works and Safety meeting dates and agenda
deadlines for your use. Please use the Engineering Department deadlines for submissions to the
Board.
Any submission to the Board requires prior approval by the Carmel Clay Plan Commission
and/or the Board of Zoning Appeals (if applicable) and completion of review by the Technical
Advisory Committee. All written requests to beplaced on the Board's agenda must inchtde the
appropriate Docket Number and the date (or dates) of approval by the Plan Commission and/or
the Board of Zoning Appeals (if applicable).

4. T.A.C. Review/Drawings submitted for approval:
We request that all comments and comment letters generated by this office be answered in writing
and be accompanied by a drawing reflecting requested revisions. Final drawings will not be
approved for construction until all Engineering Department and Utility Department issues have
been resolved. The design engineer must certify all drawings submitted for final approval. This
office will require a minimum of four-sets of drawings for approval after all issues have been
resolved. The drawings will be stamped as approved and will be signed by the City Engineer and
the Director of Carmel Utilities. The Owner will receive one-set that is to be maintained on the
construction site at all times. Carmel Utilities will receive one-set, our Construction Manager will
receive one-set and one-set will be maintained in the Engineering Department. We will approve an
additional two-sets of drawings, totaling six-sets if desired. The additional sets must be provided
with the minimum four-sets. Since there is dual jurisdiction with the Hamilton County Surveyor's
Office, the coordination of approval between the two agencies and the total number of sets of
drawings required for approval will be determined later.
5. Please be advised that any installation of signs, walls, irrigation systems, etc. within dedicated
right of way or dedicated easements will require a Consent to Encroach Agreement with the City
of Carmel's Board of Public Works and Safety.
6. Carmel Utilities should be provided drawings for review of water issues. They will provide a
separate review regarding these issues. Please assure that Carmel Utilities is on the distribution
list for all drawings and drawing revisions.
7. Carmel Utilities does not subscribe to "Holey Moley" and should be contacted directly for all
water main locations.
8. I am including copies of the following with this correspondence:

· Subdivision Building Permits
· Permit Data, Contacts, etc.
BONDING REQUIREMENTS
9. Individual Performance Guarantees will be required for the following subdivision improvements:
Streets
· Curbs
Water Mains
Storm Sewers/Drainage (dependant upon jurisdiction)
· Monuments and Markers (dependant upon jurisdiction)
· Street Signs
· Sidewalks (interior builders lot sidewalks and perimeter right of way sidewalks/right of way
sidewalks adjacent to common areas may be bonded separately).
The amount of the Performance Guarantee is based upon a Certified Engineer's Estimate for 100%
of the cost of labor and materials to construct the individual improvements, to be provided by the
design engineer. Please provide detailed Engineers Estimates for each improvement including
unit costs, quantities, materials and types of materials, etc. Upon completion and release of
individual Performance Guarantees, a three-year Maintenance Guarantee will be required. The
Maintenance Guarantee amount is based upon 15% of the Performance amount for Streets and
Curbs and 10% of the Performance amount for all other improvements. Performance Guarantees
may be Performance or Subdivision Bonds, Irrevocable Letters of Credit or
Cashier's/Certified/Official Checks. Please reference the enclosures for more detailed explanation
of our procedures.

AVAILABILITY AND CONNECTION FEES
10. Availability (acreage) Fees must be paid after all other Engineering Department requirements
have been satisfied and prior to start of construction activities (the exception is that you are
allowed to move dirt on site). Availability Fees are based upon total platted acreage for the
development at the rate of $1,010.00 per acre for Water Availability. Based upon the secondary
plat indicated acreage, the following Availability Fees would apply:
Water - 17.030 Acres @ $1,010.00/Acre = $17,200.00
Connection Fees are $1,310.00 for water per single-family residence. Connection Fees are paid
when the infrastructure has been completed, satisfactory test results obtained and the subdivision
has been released for perm/ts. Connection Fees are paid on a per-lot basis.
· If an irrigation system is planned is planned for this, or anyfitture section of the
develolmtent, additional Water Connection Fees will be assessed based upon the
recommendation of the Director of Carmel Utilities.
COMMENTS-SECONDARY PLAT
11. What is the 30' D.U.&S.E. with blank Instrument #?

11. We have engaged Crossroad Engineers to review all storm drainage plans and drainage
calculations submitted to this office for review. We will share these reviews with you as they are
received.
12. C200- Site Development Plan
· Identify the common area between Fieldshire Terrace and Amblewind Place at the north
end of the development.
· Also identify the common area on the west side of the subdivision section.
· Is there an emergency overflow outlet for Lake #7?
· Where does Lake #7 normally discharge?
· The note regarding signs, sprinkler systems, trees, etc. in the right of way should be
modified to indicate that installation of amenities will require review and approval by the
City of Carmel.
· Please indicate surface drainage swales (1% minimum slope).
13. Sheet C400, Sheet C401 - Street Plan and Profile
· We require a minimum cover distance of 12-inches, from top of pipe to bottom of pavement
section (aggregate course) on all street pipe crossings. Please place this notation on all
appropriate sheets.
14. Sheet C403 - Traffic Plan
· The Hamilton County Highway Department standards are no longer applicable. The City of
Carmel Street Department will now have jurisdiction. Be aware that the placement of all
permanent subdivision traffic control signage is governed by Ordinances under the
jurisdiction of the Carmel City Council.
15. Sheet C404 - Traffic Details
· Please delete details that do not apply.
16. Sheet C802 - Street Details
· We are revising or standard pavement section for subdivision streets as follows:
The surface requirement is not changed. 3" H.A.C. Binder #9 is changed to 5" 5D Base. 9"
Compacted Stone Sub-Base #2 is changed to 7" Compacted Stone Sub-Base #53.
Additionally, add, "All Paving is to be Completed in the Same Paving Season" to the
pavement section specification.

IH'ILITY SUMMARY FOR LAKESIDE PARK SUBDMSION
Electric service will be provided by Cinergy. The development will be served by existing
power service within the area of the subject site from the section to the south.
Telephone
Telephone service will be provided by Ameritech. The development will be serviced by
existing telephone facilities from the section to the south.
Water
Water service will be provided by Indianapolis Water Company 0WC). A watermain
must be brought to thc site from the seetiort to the south. The exact route and size will be
determined by IWC,
Sanitary Sewer
Sanitary sewer service will be provided by the Clay Township Regional Waste District
(CTRWD), CTRWD will extend the sewer from the south.
Drainage
This site is divided into three pre-developed stormwater basins. The North portion of the
site basically flows North and East to the Stultz and Almond Drain. The southeasterly
portion of the site basically flows East to the Elliot Creek Drain. The southwesterly
portion of the site flows southeasterly and ultimately flows to the East arm of the J.W.
Brendle Drain. Post-developed conditions will collect the entire development through a
series of swales and pipes directing storm water to retention ponds located throughout the
proposed development. All of the stormwater ultimately will be routed to a pond located
at the Southeast comer of the site where it will be discharged into a pipe which will cross
Tome Road, extend East on the North side of 136th Street, approximately 1350 feet
where it will mm South, cross 136~ Street and outlet into the Villages of West Clay
Subdivision and ultimately outlet into the Smltz and Almond Drain.
Gas
Natural gas service w/Il be provided by Yectren. A gas main must be extended to the site
from the south section. The exact route and size will be determined by Vectren.
