Jamie,
I have been asked to do an inspection of the Autozone on Michigan Rd in Carmel by our permitting staff. Unfortunately, I
was unable to pass the inspection due to a few issues that will need to be fixed. I have attached the approved landscape
plan for this site. Please review the site and the landscaping and ensure that the site matches the plan for the re-
inspection.
A few things to note:
• Many trees/shrubs were substituted and the changes were not approved through our office. There are three
options remove and replace the plant material with what the plan calls for, add the required species back to
these locations and leave the existing trees/shrubs as extra landscaping or submit a letter to my office stating
the change of species. I have attached a pdf that explains what is needed in the letter. This does not mean that I
will approve all the substitutions. But I will try to work with you to some point.
I recommend option one replanting the plan as approved as the tree selection was specific to the use and area,
for instance upright elms near the driveway. This would also keep the integrity of the design by the landscape
architect.
• I am not sure how long these trees have been installed, but the trunk wrap looks like it is at a point that it needs
to be removed.
• Please check all the rootballs so that they have been installed according to the tree planting detail. Normally,
the city requires the baskets to be removed. In this case this did not seem to make the plan, however, there are
many basket hoops sticking out of the ground. These should at the very least be wire cut off to remove any
tripping hazard.
Please feel free to contact me with any questions. You can email me the letter for substitutions if that is a route you
would like to take, and I will respond with an answer on what I will allow to be changed. If a meeting at the site is
appropriate just let me know.

Autozone failed the inspection. They will need to do a lot more work to pass. Do you have a contact for me to email?
Do you have the permit number. Then I can put in the inspection as a Fail.

Daren, has the Autozone at 10560 Michigan Rd. passed your inspection? It probably would have been in the summer.

The only license we require is the plumber you list on your application will need to have a state license number.

Do you have anything on paper that says if you require business licenses/registration for general
contractors to work in Carmel?

Thank you. Is this project a remodel of the building that is already at 10560 N. Michigan Rd? This application is marked new
structure but on my GIS map I’m already showing a building at that address.
