Good afternoon,
The approved storm water management permit for construction at the 5/3 Bank on Carmel Drive is attached, as well as a
new Owner Acknowledgement Agreement with the project owner's representative's signature for your records. If the
project is over 1 acre, then the Rule 5 permit may be submitted to IDEM at this time. Mark Adams has the approved Right
of Way permit.
Please schedule and hold a pre-construction meeting with Dan Greskamp before resuming construction.

Good afternoon,
I drove by the 5/3 Bank demolition today and noticed forms being installed in excavations. A storm water permit has not
be issued for the project. Only demolition has been authorized. All work related to the construction documents must stop
on site until the following is completed;
1. Final Sets have been submitted for stamp and signature.
2. The Right of Way Bond and the Storm Water Bond have been submitted.
3. The Storm Water Permit has been issued.
4. The Pre-Construction meeting has been held.
An inspector will check the site tomorrow to make sure nothing other than demolition is occuring. A formal stop work
order will be placed on the site if necessary.

Elevations are good to go (and are attached). We are okay with the slight change in them removing a window and

We have not as we are still awaiting approval from the State for the Construction Design Release. We are looking to
remove the existing building as soon as possible and will obtain additional permits while this process is commencing.

next Monday 7/16 and am having trouble getting in touch with John Thomas for obtaining SWPPP approval for
