APPROVED: The Plan Commission approved the apphcation request wilh the revisiom proposed at the August 16t~

Standards Variance approval is valid for one (1) year. By that ~e, either continuous cons~ucfion of the
improvements must be underway, or a written request for a one-time, six-month extension of the approval must have
been received and approval granted by this Department. The expiration date of the approval is August 22, 2006.

Allocation Request- Curb Cut and Open Cut for Utility Connection
Dear BPW:
I am writing this letter to formally request a commercial cub cut approval from the City of
Carmel's Board of Public Works for the oroposed project.
The site is located just south of the intersection of Main Street and Guilford Ave, on the
west side of Guilford Avenue. The site is approximately 2,15 acres. The proposed
imorovements will consist of 8 buildings with a total of 37 condominium units. There are
currently two (2) existing cum cuts for the site. We are wishing to eliminate the 2
existing outs and replace them with the proposed curb cut as shown by the attached
exhibit. The throat width of the curb cut is 22'. The radii of the curb are 25'. There is a
curb cut to the north a distance of 131' away. There are two curb cuts on the east side
of Guilford Ave. as seer on tr~e attached exhibit.
We are also seeking approval for open-cutting Guilford Avenue so that we can connect
to sanitary sewer and water service that is located along the east side of Guilford
Avenue. Please see the attached exhibit for further information.

I am writing this letter to formally request an allocation approval from the City of Carmel's
Board of Public Works for Sanitary Sewer and Water Service for the proposed project.
The site is located just south of the intersection of Main Street and Guilford Avenue, on
the west side of Guilford Avenue. The site is approximately 2.15 acres. The proposed
improvements will consist of 8 buildings with a total of 37 condominium units. There will
be twenty*two (22) 3~bedroom units and fifteen (15) 2-bedroom units. The units will be
sprinkled and there will be 2 additional fire hydrants located within the site.
