The Owner and General Contractor request a substitute 1-hr rated wall construction, in lieu of the
odginal design and cited assemblies. The walls are within 5 feet of the Property Line and assumed
to be protected both sides. See attached revised drawings.
The current East and West walls are noted as Wall Type W8.
A. Wall Description: The contractor has used a 6" -18 ga. metal stud ~ 16" OC
that is connected to the CMU base and roof/floor trusses that it bypasses(adequate
structural fastenings were installed per the engineer). The extedor face is 3 %" clay
bdck masonry, attached with metal ties every 8th course. The air gap is %~ and the
cavity is faced with an extedor '~' non-rated gypsum board screw fastened at 12"
OC. The wall will have a R-13 fiberglass batt. The interior face will be sheathed
with (2)-layers of Type "X" Gypsum Board, staggered joints, taped, and nailed at 8"
OC.
B. Reasoning for its Use: Per Table 720.4.1(2) under the last citing, a similar
steel beadng wall with veneer bdck is described with the interior using 1' cement
plaster. As this wall is rated for 2-hour it is more than adequate (need only 1-hr), but
as we do not have the inner %" sheathing rated, we feel the 2-layers of 5/8" Type 'X'
is still prudent to effect protection and integrity of the metal stud from fire on the
intedor side.
The current North or rear wall is now noted W10., original note was Wla & Wlb.

Wall Description: The contractor has used a 6' -18 ga. metal stud @ 16" OC.
that is connected to the CMU base, bypasses the second floor framing (supported
by other interior load beadng studs) and which supports the roof truss ends. The
exterior face of stud will have a 5/8" Fire Guard Dens Glass Gypsum Product screw
fastened at 8" OC) with an EIFS system applied to outside. The wall will have a R-
13 fiberglass batt. The interior face will be sheathed with (1)-layer of Type "X"
Gypsum Board, staggered joints, taped, and screwed at 8" OC.
Reasoning for its Use: Per UL U468 or Table 719.1(2) 150-1.11 or U465, we
fee! the substitution of the UL labeled Fire Guard product will meet the intent of the
assembly cited. The contractor will need to infill the "truss gaps" where the roof
trusses penetrate the assembly with studs and Type "X" 5/8"

OBSERVATIONS:
The work is in general conformance with the Design Drawings at this phase of construction.
ITEMS TO VERIFY:
. The contractor should verify and report to the engineer the connection between the wood
plate and the steel beams. The light gage wire is not sufficient or acceptable. (The bolts
between the plate and the steel beam were not installed as shown in the design
documents)
. What action has the contractor made to reinforce the foundations along the north wall?
. The contractor shall provide documentation that the wood joists are equal to those as
shown on the construction documents.

OBSERVATIONS:
. The masons were installing appropriate 3/16" brick ties and anchors.
. The bolts requested in FR 5-1 were partialiy instalied; two beams to be bolted.

ITEMS TO VERIFY:
. Brian reported that A-1 instalied two push piers at the NW and two push piers at the NE
building corners to stabilize the walls. Documentation should be provided for the owners file
(warranty information etc.)
INFORMATION OR ACTiON REQUIRED:
. Finish bolting the beam to the blocking plate
. Instali (4) #12 screws each side of second fioor window cripple into 6" steel 18 gage jamb
stud.
. Suggested that the roof vents be instalied before the second fioor ceiling drywali is
completed.
. Requested that the contractor forward documentation on the joist and trusses for the owners
file. The documents should include the design loads certification and recommendations from
the supplier for instaliation and bracing.
. Requested that blocking be instalied between joist where non-bearing walis sit paraliel to
supporting joist.
. Need to instali 3/16" dia Hilti Kwick Bolt II through track into masonry wali at 32" oc.

o (2) #10 screws between track and track top and bottom.
o Vents through roof at rear and front of building.
o Gaps in wall board between west at second floor window head height shall be filled,
. taped and spackled.
. Fire stopping at top of all walls.
ITEMS TO VERIFY:
o Flue support, flue distance to property line and distance to parapet wall.
INFORMATION OR ACTION REQUIRED:
o The fire rating on the West Wall is not completed. (Brian Jones is investigating)
o Mr. Ochs' satisfaction with the quality of the roof trusses.
ATTACHMENTS: Drawing of Detail 4 sheet A4 showing screws and attachments required. The
installation was changed from the original design.

Dear Scott,
LaFever Electric, Inc. Has made an inspection of the site listed above and will make the
following changes when we can again access the job.
I) Complete the bonding of junction boxes that had not yet been completed.
2) Install type HCF MC cable to the reccptacles and make home runs for them with type
HCF MC cable that will have equipment that will come in contact with the patients.
The above items were discussed with the inspector Jim Ochs and we were in agreement.
If you have any questions or I can be of further assistance please do not hesitate to call.
