ADDITIONAL SERVICES AMENDMENT TO
AGREEMENT FOR PURCHASE OF GOODS AND SERVICES
THIS AMENDMENT TO THE GOODS AND SERVICES AGREEMENT ("Agreement") entered into by and
between the City of Carmel and Wilkinson Brothers (the "Vendor'), as City Contract dated January 22, 2015 shall
amend the terms of the Agreement by adding the additional services to be provided by Vendor consistent with the
Scope of Work attached hereto and incorporated herein as Exhibit "A". Furthermore, the terms of the Agreement
shall be amended to include the E -verify requirement as stated in Indiana Code § 22-5-1.7 et seq., which is attached
hereto and incorporated herein as Exhibit "B". as well as the required E -verify Affidavit, attached hereto and
incorporated herein as Exhibit "C". Also, pursuant to I.C. § 5-22-16.5, Vendor shall certify that in signing this
document, it does not engage in investment activities within the Country of Iran. The terms and conditions of the
Agreement shall not otherwise be affected by this Additional Services Amendment and shall remain in full force and
effect.

RE: City of Carmel Request for Additional Services
This document outlines the additional professional services requested by the City of Carmel for services that
include coordinating, designing and managing the production of the Indianapolis Airport advertising as well as
additional creative services for advertising and promotions for the City of Carmel and Carmel Arts & Design District.
COST for DESIGN, PRODUCTION, & DEVELOPMENT (Additional Services): $20,000
Should our duties require less than the above anticipated cost, we will adjust the amount accordingly.
The cost will not exceed this amount.
COST NOTES:
• All design, additions or updates are estimated at a rate of $75.100 per hour (depending on skill sets required),
based on the previous years of production on event -related projects.
• The costs above include any hosting or registration and vendor expenses necessary.
PAYMENT TERMS:
Payment will be requested as work is completed and is not anticipated to exceed a total of $20,000 for the
remainder of 2015. This quote is based on information presented by the City of Carmel. We will gladly follow
the pay schedule of the City of Carmel.
