IU Health/HR/2nd Amendment for Master Services AgreementService Line Attachment #1� 1. Service Model: A healthcare clinic for participants in the City of Carmel Employee Health Benefit Plan, with dedicated medical staff providing primary care, preventive care, wellness education and related services. Staff will include the following: • IU Health Physician — Twenty one (2 1) hours per week • IU Health Nurse Practitioner— Twelve (12) hours per week • IU Health Medical Assistant —Thirty three (33) hours per week • IU Health Nurse Manager - Thirty three (33) hours per week • IU Health Health Coach — Twelve (12) hours per week 4. Cost of Services/Billing Services shall be billed to City in accordance with the following fee schedule. Services shall be invoiced monthly with payment due within thirty (30) days of receipt of billed invoice. Hourly rates for staff include the following: • Staff recruitment and retention • Staff education, insurance and oversight • Support services for staff, including payroll, information technology, legal and human resources • Computer hardware • Online scheduling system • Electronic medical recordkeeping system • Telephones and telephone lines • Office equipment, including printer, scanner, and facsimile machine • Clinic communication and marketing program Rate Increases: An annual cost -of -living -adjustment (COLA) will be made for staff rates beginning on 1/1/2018 upon review and agreement of the parties not to exceed 4% in any one year period. Indiana University. -Health Workplace Services 2nd AMENDMENT TO CITY OF CARMEL, INDIANA MASTER SERVICES AGREEMENT FOR DESIGNATED HEALTH CARE SERVICES This Amendment, effective as of January 1, 2017 amends the City of Carmel, Indiana Master Services Agreement for Designated Health Care Services dated February 1, 2013, (the "Agreement"), by and between City of Carmel, Indiana (hereinafter "CITY") and III HEALTH WORKPLACE SERVICES (hereinafter "IU HEALTH"). WHEREAS, the parties wish to amend the Service Line Attachment #1 to include an annual Cost of Living Adjustment (COLA) and change the work hours for set -vices performed by the Physician (MD) and Nurse Practitioner (NP). NOW, THE, the parties agree as follows: Service Line Attachment #1, Section 1 and Section 4 shall be amended as attached hereto and incorporated herein and shall become effective on the Effective date. This amendment reflects three changes to the pay and hours of clinic staff. 1) The Nurse Practitioner is working four (4) hours more per week and the Physician four (4) hours less. This change is due to scheduling conflicts that arose when we expanded the clinic hours in 2016. 2) The clinic staff is being given pay increases ranging from 2.7% — 3.2%. This will be the first increase Dr. Fagin has received since the clinic opened in May 2013. 3) Language has been added to allow for annual cost -of -living adjustments upon review and agreement of both parties.
