ADDITIONAL SERVICES AMENDMENT TO
AGREEMENT FOR PROFESSIONAL SERVICES
THIS AMENDMENT TO THE AGREEMENT FOR PROFESSIONAL SERVICES ("Agreement") entered into
by and between the City of Carmel and Bartlett Tree Experts (the "Professional"), as City Contract dated March 15,
2017 shall amend the terms of the Agreement by adding the additional services to be provided by Professional
consistent with the Scope of Work attached hereto and incorporated herein as Exhibit"A". The terms and conditions
of the Agreement shall not otherwise be affected by this Additional Services Amendment and shall remain in full force
and effect.

program,Bartlett Tree Experts recommends having a qualified arborist inspect your property periodically to assist you in Identifying
potential risks or hazardous conditions relating to your trees or shrubs. THIS IS NOT AN INVOICE.
Thank you for continuing to call on us for your tree and shrub care needs. If we have overlooked
any of your immediate concerns, please let me know.
Provide arboricultural consulting services for Main St Tree Pit Planning located at
Department of Community Services. Cost not to exceed $130 per hour.
• Note- provide up to(10)hours of consultative services to assist in planning for tree pit
installations as designated by client. Consultation may Include, but not limited to: Site
visits, procedural recommendations, estimated costs,future tree maintenance needs,
etc.
Amount:$1,300.00
