Provide complete description and ingredient list of each item (including allergens) to St. Vincent Carmel a minimum of 48 hours prior to delivery. Email to monica.evans@ascension.org 2. Assurance that restaurant staff have all followed proper food safety and handling procedures when preparing food and also throughout the delivery and receiving process. 3. Deliveries: a. Arrive at 12pm (as close to the time as possible) and then call 765-438-5786 to alert the kitchen to come receive the food. b. Go to entrance 1 at the Carmel St. Vincent Hospital which is located at 13500 N. Meridian St (Entrance off of Old Meridian. c. Delivery staff should wear gloves and mask. 4. All food is to be individually pre-packaged before delivery and labeled with an appropriate expiration date. 5. If questions arise – please call Monday-Friday business hours, Monica Evans.
