THIS AGREEMENT, executed to be effective the first day of July, 2000, by and between
CoreSource, Inc., a Delaware corporation, hereinafter referred to as the "Plan Supervisor", and City of
Carreel, hereinafter referred to as the "Company"
WITNESSETH:
WHEREAS, the Company heretofore established and adopted the Plan Supervisor Agreement for
City of Carmel to establish guidelines which state the duties and responsibilities of both the Company and
the Plan Supervisor, and
WHEREAS, the Company desires to amend said Plan Supervisor Agreement as set forth:
NOW, THEREFORE, in consideration of mutual promises of the prospective parties hereto, and hi
order to amend the Plan Supervisor Agreement for City of Camel, the parties do hereby amend the Plan
Supervisor Agreement as follows:
1.
Fee Schedule
Increase in Sagamorn Access Fee $3.05 per participant per month.
2. Premiums shall be listed on contract between: carrier and company.
3. In all other respects, the Plan Supervisor Agreement shall remain unchanged.
