This letter gives approval to Ramsey Pyrotechnics, Inc. CRamsey") to launch fireworks at the Camel City
Center on November 7, 2000 between the hours of 7:00 A.M. and 7:00 p.m.
You have indicated you will furnish all equipment, materials, personnel and permits necessary to fire a
display in accordance with NFPA 1123, local and state regulations. You have also indicated you would
remove all unexploded and dangerous materials and furnish a certificate of insurance to the City of Carn~el.

On November 1, 2000, the Board of Public Works and Safety approved a contract to do business
with your company. Enclosed is a fully executed copy of that contract for your records.
Please review the enclosed sample invoice (Exhibit B). You do not need to use this specific
form, but we require you to submit the requested information in a similar layout. This format
replaces any other form previously used by the City and is effective upon approval of your
contract.
