RIDER TO PROFESSIONAL SERVICES AGREEMENT
This Rider to the Professional Services Agreement ("Agreement"), executed by Richard Heck ("Professional") and
entered into by and
the City of Carmel, Indiana ("City") on December 15, 1999 (City Contract No. 12.15.99.06), js_jaereby MO.fi/
between the City, acting through its Board of Public Works and Safety, and Professional on this I ~"'day of
2002.
WHEREAS, the Agreement contains a professional responsibility insurance requirement that the City, upon request,
typically waives in contracts such as this and now desires to waive in this case.
NOW, THEREFORE, THE PARTIES AGREE AS FOLLOWS:
1. The above Recitals are incorporated herein as though they were fully set forth below.
2. City hereby waives the professional responsibility insurance requirements set forth in Section 7.5 of the Agreement
and in Exhibit C attached thereto.
3. This Rider shall continue in effect from year to year, unless otherwise modified or terminated in writing by the
parties hereto, except that this Rider shall automatically terminate on the date on which the Agreement itself is terminated.
4. The terms of this Rider shall supersede any contrary terms or conditions contained in the Agreement. All
Agreement terms and conditions not so affected remain in full force and effect, are incorporated herein by this reference and
shall apply to this Rider as well.
5. The effective date of this Rider shall be the date on which it is last signed by a party hereto.
