Amendment 1-02 to Engineering Service Agreement
Dear Mr. Duffy:
The Engineering Service Agreement dated November 3, 1999 is amended as follows:
Jones & Henry Engineers, Ltd., will provide the following engineering services to the City of Carmel
in connection with the design and construction of water transmission mains.

Prepare drawings and specifications for approximately 9,400 If of 12-in. water main along
141't Street.
Prepare drawings and specifications for approximately 15,200 lfof20-in. water transmission
mains from 146th Street to Hamilton Western Utilities booster pump station,
Design improvements to Hamilton Western Utilities booster pump station to increase
capacity.
Design a meter vault at Springmill and 146m Street to meter water from or to the Town of
Westfield's water distribution system.
Assist the OWNER during bidding and construction of the above projects.
The OWNER shall pay the ENGINEER for the services listed above in accordance with Appendix
A of the Engineering Service Agreement dated November 3, 1999. The total cost will not exceed
$170,000 without prior notification.
The above Scope of Services and Cost of Services are agreed to by:

Amendment 3-02 to Engineering Service Agreement
Dear Mr. Duffy:
Jones & Henry Engineers, Ltd., will provide the following additional engineering services to the City
of Carmel in connection with Emerson Road Sewer Rehab Project.
Estimate the construction costs for pipe bursting versus lining the main sewer and laterals on
Emerson Road, York Drive and Park Lane.
Prepare specification and bid documents for either pipe bursting or lining the sewer mains and
laterals.
The OWNER shall pay the ENGINEER for the services listed above in accordance with Appendix
A of the Engineering Service Agreement dated November 3, 1999. The total cost will not exceed
$18,000 without prior notification.
The above Scope of Services and Cost of Services are agreed to by:

The Engineering Service Agreement dated November 3, 1999 is amended as follows:
Jones & Henry Engineers, Ltd., will provide the following engineering services to the City of Carmel
in connection with the design and construction of an elevated water storage tank.
Develop a conceptual design of a 2 MG composite elevated water storage tank showing
interior and exterior architectural features.
Develop drawings and specifications suitable for bidding the construction of the elevated
water tank.
Assist the OWNER during bidding and construction.
Provide periodic specialty inspection services by Dixon Engineers.
The OWNER shall pay the ENGINEER for the services listed above in accordance with Appendix
A of the Engineering Service Agreement dated November 3, 1999. The total cost will not exceed
$100,000 without prior notification.
The above Scope of Services and Cost of Services are agreed to by:
