EXECUTIVE ORDER #99-1
Effective January 1, 2000, Frank E. McKain of Conseco Risk Management is appointed
agent of record for the City of Carmel's property and casualty, liability and workers
compensation insurance. This appointment supersedes all previous appointments and is
effective until revoked in writing.
