accessories, paper goods and other items (Products) that are used in connection with Equipment at such 1

3. Title and ownership to the Equipment shall remain in Calderon.
4. Owner authorizes Calderon to enter the Location for the purpose of servicing, inspecting, changing,
removing or repossessing the Equipment. Owner agrees that the Equipment shall not be moved from the
Location CO any other place without the prior written consent of Calderon. Owner agrees not to pax
rteglea nor tamper with the Equipment nor permit the same CO occur and Owner does agree to be responsible for
any damages that may occur to the Equipment while at the Location, normal wear and tear excepted. Comet

5. In order to induce Calderon to extend credit to Owner, the undersigned (Owner) hereby agrees to

In the event that an action is commenced, Owner hereby authorizes to the jurisdiction of
the Courts of Marion County, in the state of Indiana.
6. If during the term of this Agreement all or part of the business conducted at the Location shall be
moved to a new site,this Agreement,at Calderon's option, shall be deemed amended to apply to the new site.
7. This Agreement shall be automatically extended for successive periods of one year each unless
either party shall give the other written notice at least thirty (30) @j days prior to the expiration of the
then current period, provided. Eidtkefax however, that either party may terminate this Agreement without
cause or further obligation upon at least ten (10) days prior written notice to the other party inliducgt:Odtftlith

. immediately, without notice, if the Location is partially or totally destroyed or if the business conducted
therefrom is stopped or materially diminished.
S. Upon termination or expiration of this Agreement, Owner shall return the Equipment in good
condition,normal wear and tear excepted.
9. This Agreement contains the entire Agreement between the parties and there are no other
understandings, agreements, or representations between them or if there are, then this Agreement supersedes
them. This Agreement shall not become binding on Calderon until executed by an authorized agent or until the
Equipment is furnished, whichever occurs first. This Agreement is binding upon the heirs, successors and
assigns of the Etivilmactltic
10. Owner agrees to hold Calderon harmless from any losses, liabilities, damages, costs and expenses
incurred by or imposed against Calderon to the extent the same are caused by the negligence of Owner.
11. Other Agreements.
Calderon agrees to comply with all applicable laws during its provision of services pursuant to this Agreement.
