DLZ/Engr (2) DLZ Engineering Department 2010 Appropriation #202 -509; P.O. #27451 Contract Not To Exceed $33,500.00 ADDITIONAL SERVICES AMENDMENT TO APFROV`O AS TO FORM BY5 AGREEMENT FOR PURCHASE OF GOODS AND SERVICES THIS AMENDMENT TO THE GOODS AND SERVICES AGREEMENT "Agreement") entered into by and between the City of Carmel and DLZ (the "Vendor"), as City Contract #050510.15 shall amend the terms of the Agreement by adding the additional services to be provided by Vendor consistent with the Scope of Work attached hereto and incorporated herein as Exhibit "A The terms and conditions of the Agreement shall not otherwise be affected by his Additional Services Amendment and shall remain in full force and effect. PLEASE NOTE Please be advised that "Additional Services" performed on City contracts must be billed separately from the original contract and other additional services. More importantly, the invoice must reference the appropriate Additional Service and P.O. shown above. Invoices received without the Additional Service referenced will be returned for proper identification. PTOE Subject: Additional Services Pursuant to your request via e -mail dated September 09, 2010, this memorandum serves as a request for additional funding approval to conduct traffic counts at the remainder of the intersections included in the DLZ's scope of services dated, October 2009 (Scope). This Scope included Task A Traffic Data Update that comprised of two sub -tasks as follows: 1. Intersection data collection (total of 160 intersections for a fee of $150,500.00) 2. Roadway segment data collection (total of 29 segments for a fee of $21,500.00) For Item #1 above (Intersection data collection), the City previously authorized $117,000.00 via PO 21384 for up to 120 intersections only. Please refer to the Memorandum, dated March 19, 2010 and subsequent contract dated May 05, 2010. Based on the afore- mentioned Scope, DLZ requests the authorization of the remainder funds in the amount of $33,500.00 to complete Intersection data collection of all 160 intersections, as stated above. This does not include the Roadway segment data collection or any other service services stated in the Scope, since these services are not desired by the City at this time. Thank you. cc.
