ADDITIONAL SERVICES AMENDMENT TO
AGREEMENT FOR PURCHASE OF GOODS AND SERVICES
THIS AMENDMENT TO THE GOODS AND SERVICES AGREEMENT "Agreement entered into by and c� 1.1
between the City of Carmel and Plymate's Matman (the "Vendor as City Contract 08 "18 shall amend the
terms of the Agreement by adding the additional services to be provided by Vendor consistent with the Scope of
Work attached hereto and incorporated herein as Exhibit "A The terms and conditions of the Agreement shall not
otherwise be affected by his Additional Services Amendment and shall remain in full force and effect.

The service includes delivery of clean mats and pick up of the soiled mats every other week. The mats are
guaranteed to present a consistent high image every time. We inspect the mats for rips and tears and make
every effort to eliminate those from our inventory. Your delivery will be timely each and every delivery.
