ADDITIONAL SERVICES AMENDMENT TO APPt1OVZO AS TO FORM BY
AGREEMENT FOR PURCHASE OF GOODS AND SERVICES
THIS AMENDMENT TO THE GOODS AND SERVICES AGREEMENT ("Agreement") entered into by and
between the City of Carmel and A&F Engineering (the "Vendor), as City Contract # 040710.08 shall amend the
terms of the Agreement by adding the additional services to be provided by Vendor consistent with the Scope of
Work attached hereto and incorporated herein as Exhibit°A". The terms and conditions of the Agreement shall not
otherwise be affected by his Additional Services Amendment and shall remain in full force and effect.

As we discussed,the City of Carmel has a need for additional professional services and pursuant to our current
contract with A&F, Contract#040710.08, dated April 7, 2010, I have requested and received from you the proposed
scope of services and fee estimates for the following work task:
Additional Service Amendment#1— Engineering Services for HSIP funding
116°i & Gray, and 111th and Pennsylvania
See "Attachment A" Not to Exceed:$16,255.00
Per our contract procedural requirements, I hereby approve these Additional Professional Services estimates, and
request that the Board of Public Works ratify the above referenced Contract Amendment.
PLEASE NOTE
Please be advised that"Additional Services"performed on City contracts must be billed separately from the original
contract and other additional services. More importantly,the invoice must reference the appropriate Additional
Service#and P.O. # shown above. Invoices received without the Additional Service #referenced will be returned

Dear Mr.McBride:
This letter will serve as our proposal and fee justification for the above-referenced project.
The scope of services will be:
• Prepare a preliminary sketch design for a roundabout design at 116t1i Street&Gray Road to determine how
and if the roundabout will geometrically fit at this location.
• Prepare a cost benefit analysis for the roundabout(both intersections).
• Prepare preliminary construction estimate for the roundabout(both intersections).
A&F Engineering previously completed a preliminary design for the roundabout at 111t1i Street and Pennsylvania
Street and will use this design to complete the analysis and estimate.
The hours to prepare this work will be:

Traffic Manager—Design 10.0 hours 4.0 hours
Project Manager 50.0 hours 8.0 hours

The lump sum fee to prepare this work is$12,370.00 for 116`1i Street and Gray Road and$3,885.00 for 111'1i Street
