AGREEMENT FOR PURCHASE OF GOODS AND SERVICES

THIS AMENDMENT TO THE GOODS AND SERVICES AGREEMENT ( "Agreement ") entered into by and
between the City of Carmel and Murrey & Trettle, Inc., (the "Vendor"), as City Contract # 01.17.07.01 shall amend the
terms of the Agreement by adding the additional services to be provided by Vendor consistent with the Scope of
Work attached hereto and incorporated herein as Exhibit "A ". The terms and conditions of the Agreement shall not
otherwise be affected by his Additional Services Amendment and shall remain in full force and effect.

Dear David,
As we discussed, the City of Carmel has a need for additional professional services and pursuant to our
current contract with Murray and Trettel, Contract 01.17.07.01 dated January 17th, 2007. I have
requested and received from you the proposed scope of services and fee estimates for the following
work task:
Additional Service Amendment #1- Advanced Weather Forecasting Service
See "Attachment A" Not to Exceed:$2,200
Per our contract procedural requirements, I hereby approve this Additional Professional Services
estimate, and request the Board of Public Works ratify the above referenced Contract Amendment.
PLEASE NOTE
Please be advised that "additional Services" performed on City contracts must be billed separately from
the original contract and other additional services. More importantly, the invoice must reference the
appropriate additional service # and P.O.# shown above. Invoices received without the Additional
Service # referenced will be returned for proper identification. Thank you for your continued assistance
in providing the City of Carmel with your professional, quality services. If you should have any

It's that time of year again.
We are planning to offer you the same services at the same cost as last winter.
While it took awhile for all of us to recover from another unusually harsh and busy winter last
season, we are getting geared up for the winter ahead. Last winter we experienced another winter
with above normal snow and above normal snow events.
Attached, please find the Contact Data Form that will assure that we get the vital information to
the appropriate person or office again this winter.
Due to the increasing demand for email and personal electronic device communication, we have a
favor to ask. If our Warnings and/or messages go to more than 5 people, we ask that your
organization create a single email address that will facilitate delivery to people in your
organization.
Murray and Trettel offers other winter weather services that your organization might find useful
in dealing with Old Man Winter. Our team of experienced meteorologists works 24/7 to
provide geographically specific warnings and forecasts that allow you to make cost effective
decisions.
