THIS AMENDMENT TO THE GOODS AND SERVICES AGREEMENT ("Agreement") entered into by and
between the City of Carmel and American Structurepoint (the "Vendor'), as City Contract#060210.04 shall amend
the terms of the Agreement by adding the additional services to be provided by Vendor consistent with the Scope of
Work attached hereto and incorporated herein as Exhibit "A". The terms and conditions of the Agreement shall not
otherwise be affected by his Additional Services Amendment and shall remain in full force and effect.

Additional Services Request

Dear Mr. McBride:
On behalf of American Structurepoint, Inc., I am submitting this request for authorization of
additional professional services for the Chesterton Subdivision Drainage Improvements Project.
The additional professional services shall include additional topographic survey of sanitary
laterals day lighted by the private utility locator and private utility locator fee for pot holing and
locating additional sanitary sewer laterals. When contacted by the City of Cannel, American
Structurepoint will complete these services at the hourly rates set forth in the original contract.
Our fee for the additional services will not exceed $9,500 without written agreement by both
parties. Because the previously authorized services for land appraisal on this project are no
longer required, the unused balance of this fee ($2,940) can be applied to the additional services
described in this request. Therefore, the net increase in fee requested under this task order is
$6,560. Invoices will be forwarded to the City of Carmel on a monthly basis in accordance with
our original contract.
