Dear Barbara,
VSP has reviewed your program and developed rates based on the experience of the overall plan
utilization. Your Plan Term expires on May 1, 2011, and we are pleased to offer you another period of
quality coverage at your current rates.
From eyewear selection to provider locations, choice is important for your members. That's why I'm
excited to let you know about VSP Open Access which gives members the flexibility to use their
VSP benefits at any location, including specialty optical boutiques and retail chains. The Open
Access out -of- network schedule is enhanced with increased reimbursement amounts is included
with your renewal. VSP Open Access also makes it easy for members to choose the eyewear brands
they want from the optical location that works best for their circumstances. Members will still have
exclusive access to the VSP Network, the largest and best network of independent eyecare
providers.
To renew your contract, please have the enclosed Renewal Agreement signed and return it to VSP
via scanned email or fax 317 686 -1140 by March 15, 2011. VSP will produce your renewal contract when
we have received the Signed Renewal Agreement. Please review the new contract carefully, since
some of the provisions may have changed from your prior contract. Additionally, please keep a copy
of this letter and Renewal Agreement, given that they serve as your Notice of Renewal.

To renew your contract and maintain continuous service, please select a plan option, sign and return the Renewal
Agreement via fax at 317 686 -1140 by March 15, 2011. VSP will produce your renewal contract when we have received
the Signed Renewal Agreement. Please review the new contract carefully, since some of the provisions may have
changed from your prbr contract. Additionally. please keep a copy of this Renewal Agreement and accompanying
letter, iven that they serve as your Notice of Renewal. If you should need communication materials to explain any
new benefit levels or rates, please provide the signed renewal and allow a minimum of three weeks notice. Thank
