To that extent, we have reviewed the proposed project and have anticipated the necessary scope of work for the project in order to provide professional services. SCOPE OF WORK Based upon information provided by yourself and review of information readily available on line, Cripe Architects Engineers understands the current scope of work to be as follows: Box Culvert Extension /Replacement: Creation of construction documents associated with the replacement of the existing box culvert located on River Road between Tall Timber Run and Haverton Way. The box culvert needs replacement due to its aging condition and structural integrity. As part of the design, it will be lengthened to support for the future road improvements and capacity will remain the same to not create potential issues with raising the 100 year flood elevation in the White River. It is our understanding there are two (2) 36" water mains in the vicinity that the design will need to be protect from disturbance and /or relocation. Additionally, it is anticipated the box culvert will be a "CON /SPAN" type structure and structural details will be provided by the vendor. These efforts will include submittal to the governing authorities to obtain project approval, bidding assistance, and support services through construction. Exhibit Y C npe -Nitaurv. Architects Engineers Solutions by Design Since 1937 Telephone 317 844 6777 Facsimile 317 706 6464 3939 Priority Way South Drive, Suite 400 Indianapolis, Indiana 46240 www.cripe.biz River Road Schematic Design: Associated with the Box Culvert scope of work, Cripe Architects Engineers will prepare a conceptual level design for future road improvements (from Tall Timber Run and Haverton Way) based upon the previously prepared Prairie View Plans as designed by Schneider. It is our understanding this portion of the road improvements were designed, but not implemented. We will review the design, and make changes as appropriate. It is anticipated the horizontal alignment will not change, but vertical alignment may due to the replacement of the box culvert. The deliverable for this effort will be a plan and profile of River Road as well as a rendered presentation drawing in plan view mounted on presentation board. This conceptual layout and presentation drawing will then assist the City of Carmel with securing funding for the implementation of the road improvements. This scope of work is for schematic design layout only, but Cripe Architects Engineers would be happy to assist the City of Carmel with this project when it is implemented. White River Trail: As part of the box culvert replacement /extension, the White River Trail will be extended from the extension trail on the south termination (approximately at Tall Timber Run) to the intersection of Haverton Way and River Road along the east side of River Road. These efforts will include submittal to the governing authorities to obtain project approval, bidding assistance, and support services through construction. Pedestrian Improvements at Haverton Way /River Road: Based upon the sketch provided by yourself on 4/19/11, pedestrian improvements at the entrance of Haverton Way will be implemented based conceptually on the provided sketch. The goal will be to provide a pedestrian striped cross walk with median islands for refuge, as well as some nominal sidewalks at the intersection of Lost Oaks Drive and Haverton Way. The improvements will included ADA access as applicable. These efforts will include submittal to the governing authorities to obtain project approval, bidding assistance, and support services through construction. A Mr. Gary Duncan City of Carmel y April 27, 201 1 Page 2 t, C ripe 0 Architects Engineers Solutions by Design Since 1937 Telephone 317 844 6777 Facsimile 317 706 6464 3939 Priority Way South Drive, Suite 400 Indianapolis, Indiana 46240 www.cripe.biz Based upon this understanding, Cripe Architects Engineers has developed the following detailed Scope of Services to be rendered: ENGINEERING DESIGN SCOPE OF WORK 30% Construction Documents During this phase, Cripe Architects Engineers will perform the topographic survey, and layout the improvements in plan view and profile review for a meeting with the City of Carmel for review and discussion. Due to the goal of starting construction as soon as feasible, it is recommended that Cripe Architects Engineers then be released to completed 100 %o Construction Documents. for submittal to the permitting for approval. Once permit comments are received, we will proceed with incorporating the permit comments into the plan set as well as any final City of Carmel comments. This plan set can then be returned to the permitting agencies, as well as put out to bid. Construction Documents Cripe Architects Engineers will prepare one (1) set of construction plans and front end /technical specifications outlining the specific requirements of the project components (Box Culvert Extension /Replacement, White River Trail, and Pedestrian Improvements at Haverton Way, River Road. The following is the list of the anticipated Civil Engineering activities and construction documents that will be included within the Construction Document Phase of the Scope of Services for the proposed project: Construction Details /Geometric plans shall be prepared to show horizontal control of site improvements, demolition items required, and utility relocations as required. Profiles indicating vertical alignment and grade of White River Trail and Box Culvert will be provided. Erosion control plan and associated notes and details will be prepared for control of erosion during and after site construction to serve as the stormwater pollution prevention plan. Details shall be prepared for critical design /construction features. Signing and Striping Plans shall be provided for reconstruction of River Road as required due to Box Culvert. Maintenance of Traffic plans shall be provided. It is our understanding the construction of the box culvert will be performed under a road closure. Drainage Report will be prepared for permit approval. Operations and Maintenance Manual for Best Management Practices will be prepared for permit approval. Based upon our research and anticipated scope of work, the following submittals will be required: Submittal to the City of Carmel for review and approval of Construction Documents. Submittal to County Surveyor for review and approval of Box Culvert Extension /Replacement. Submittal to the Indiana Department of Environmental Management for NPDES Notice of Intent for erosion control. Submittal to the US Army Corps of Engineers for a Wetland 404 Permit Submittal to IDEM for a 401 Water Quality Certification for creek relocation. Submittal to DNR for Floodway /Floodplain permit. Submittal to the utility companies for coordination purposes if relocations are required. All submittal and permit fees shall be paid by the City of Carmel. BIDDING It is our understanding, this project will be constructed with 100% local funding. Cripe Architects Engineers will assist with the following public bid process activities: Advertisement for bids; Hold the Pre -bid conference to address contractor technical questions; Issue Addendums to address contractor questions; Preparation of bid tabulations and bid review; Conduct Bid Opening; Recommendation of lowest qualified bidders. It is also assumed the bid will be unit prices therefore requiring the preparation of Bid Tabulations. It is also assumed the City of Carmel will require an Engineer's Opinion of Probable Costs be prepared by Cripe Architects Engineers. These visits are not construction inspection services; but rather, for observation purposes only. It is our understanding the City of Carmel will provide a full time inspector for this project at the time of construction. Cripe Architects Engineers welcomes the opportunity to assist you with this effort. Professional fees for construction inspection can be provided at your request, but have not been included in this scope of work. Relevant job site questions and Requests for Information from the contractor, Owner or design team will be addressed as well as the review of submittals. Site visits and /or progress meetings beyond the aforementioned shall be considered additional services and will be invoiced to the City of Carmel at Cripe Architects Engineers' current hourly rates. ENGINEERING ASSUMPTION /EXCLUSIONS The above Scope of Services was based on the following assumptions and /or exclusions: It is recommended the Owner /Client retain the services of a geotechnical engineer to provide site specific soil conditions for the design of the Box Culvert. Additionally, an environmental engineer may be prudent to check for potential impacts for the White River Trail. Copies of existing construction documents that have been prepared for River Road will be provided to Cripe in Autocad, as well as Drainage Reports that support the drainage in this area if applicable. Cripe Architects Engineers will assumed five (5) meetings with the City of Carmel to review the construction documents, and three (3) meetings with agencies for review of project for approval /permitting. If these assumptions prove not valid, Cripe Architects Engineers may request an additional services contract to perform any additional scope of work requested by the Owner. The topographic survey will be performed in accordance with current mapping standards, and will include linework of existing improvements, locations of utilities, contours at 1' intervals, and relevant spot elevations. Cripe will provide copies, both electronic and paper, of the survey for the project site, and will initiate these services upon receipt of written notice to proceed on the project. LAND SURVEYING ASSUMPTIONS /EXCLUSIONS The above Scope of Services was based on the following assumptions and /or exclusions: The proposed survey area is limited to the area defined by the solid yellow perimeter line in the attached exhibit. The survey will be based on city- approved datum It is assumed that existing survey control monuments will be found in close proximity to the subject site, in good condition, and within accepted tolerances for relative positional accuracy. Road and street right -of -way lines will be re- established based on best available evidence and record documents. Complete right -of -way engineering is not included. No new boundary or right -of -way monuments will be set, and no new legal description will be written. Cripe survey crews shall have free access (right of entry) to the entire property, and the survey shall not be delayed by vehicles, pedestrians, or hazardous conditions. Cripe Architects Engineers will contact the Indiana Underground Plant Protection Service "Holey Moley to mark the location of buried utilities within the right -of -way. Only those markings, and above ground visible evidence of utilities will be located on this survey. Construction staking is not included. As -built surveying of new improvements is not included. All proposed delivery schedules and dates are subject to weather conditions. If these assumptions prove not valid, Cripe Architects Engineers may request an additional services contract to perform any additional scope of work requested by the Owner.
