AGREEMENT FOR PURCHASE OF GOODS AND SERVICES IA
THIS AMENDMENT TO THE GOODS AND SERVICES AGREEMENT ("Agreement") entered into by and
between the City of Carmel and (the "Vendor"), as City Contract # 040710.08 shall amend the terms of the
Agreement by adding the additional services to be provided by Vendor consistent with the Scope of Work attached
hereto and incorporated herein as Exhibit "A". The terms and conditions of the Agreement shall not otherwise be
affected by this Additional Services Amendment and shall remain in full force and effect,

PROPOSAL FOR
SPEED STUDIES
CITY OF CAMEL
CARMEL, INDIANA
AUGUST 2011
The purpose of this project is to make speed limit recommendations along several roadway
segments within the study area. The study area and scope of the study is defined as follows.
STUDY AREA:
The study area for this analysis is bounded by 96th Street, Shelbourne Road, 1 16`h Street &
Springmill Road.
I. The study roadways will include:
a. 96`h Street (Within boundaries described above and sites near the Monon Trail
Crossing)
b. I 06th Street
c. 116'h Street
d. Shelbourne Road
e. Towne Road
f. Ditch Road
g. Springmill Road
SCOPE OF WORK:
The scope of work for this project is:
1. Conduct roadway geometric inventories along the study area roadway segments.
2. Conduct existing posted speed limit inventories along the study area roadway segments.
3. Collect roadway segment volume and speed data along the study roadways using
pneumatic traffic data collection devices.
4. Prepare a report documenting all existing data, analysis and recommendations regarding
posted speed limits along the study roadways.

FEES:
The fees to provide the services outlined in the scope of work are as follows:
I, The lump sum fee to provide the services outlined above, is $12,285.00. All meetings
will be billed based on the hourly rate schedule below.
2: The fees to provide additional services, if required, will be billed on the basis of an
hourly rate as shown in the hourly rate schedule. However, a lump sum or maximum fee
will be offered if the scope of work for any additional services can be defined.

d. Designer $100.00
