As we discussed, the City of Carmel has a need for additional professional services and pursuant to our current
contract with CrossRoad Engineers, Contract #030310.03 dated March 3, 2010, I have requested and received from
you the proposed scope of services and fee estimates for the following work task:
Downtown Carmel Intersection /Street Evaluations
Engineering Services
See "Attachment A" Not to Exceed: $18,500.00
Per our contract procedural requirements, I hereby approve these Additional Professional Services estimates, and
request that the Board of Public Works ratify the above referenced Contract Amendment.

Please be advised that "Additional Services" performed on City contracts must be billed separately from the original
contract and other additional services. More importantly, the invoice must reference the appropriate Additional
Service and P.O. shown above. Invoices received without the Additional Service referenced will be returned

THIS AMENDMENT TO THE GOODS AND SERVICES AGREEMENT (`Agreement entered into by and
between the City of Carmel and CrossRoad Engineers, P. C. (the "Vendor as City Contract 030310.03 shall
amend the terms of the Agreement by adding the additional services to be provided by Vendor consistent with the
Scope of Work attached hereto and incorporated herein as Exhibit "A The terms and conditions of the Agreement
shall not otherwise be affected by this Additional Services Amendment and shall remain in full force and effect.

CrossRoad Engineers is pleased to present this Scope of Services and Fee Proposal to provide
professional services associated with the evaluation of sight distances at the following
intersections:
3rd Street NE and Rangeline Road evaluate for parking restrictions on 3 Street NE (if
cars parked on one side or both, traffic turning off of Rangeline has to yield)
6 Street NW and Rangeline Road evaluate for parking on 6 Street NW (if cars parked
on one side or both, traffic turning off of Rangeline has to yield)
5 Street SE and Rangeline Road evaluate for sight distance on Rangeline Road
1st Street SW and Rangeline Road evaluate for sight distance on Rangeline Road
1 Street NW and 3 Ave NW evaluate for parking on 3 Ave NW. Provide
recommendation on striping plan for the current on street parking areas as well as a
recommendation on the elimination of the northernmost parking space on the east side of
3 Avenue at 1 Street NW.
1 Street NW and 2 Ave NW evaluate for parking near intersection
1 Street NW and 1 Ave NW evaluate for parking near intersection
Main Street and l st Ave SW evaluate for parking near intersection
1 Street SW and 3 Ave SW evaluate for parking near intersection
2 Ave NW evaluate for being a one way with parking on one side north of 1s Street
NW or parking restrictions on one side
3 Ave SW evaluate width and for parking restrictions on one side between Main St
and l St SW
1 St SW evaluate width and for parking restrictions on one side between 4 Ave SW

Evaluate sight lines at the intersection of Main Street and 1 Avenue NW and 1 Avenue
SW due to the new building and on- street parking on the north side of Main Street and, if
any, parking on 1 Avenue NW.

Our fee for this work will be a lump sum of 1 8,500.
We will utilize aerial maps in conjunction with field check information to evaluate these
intersections and provide diagrams with sight distance required for the posted speed limit and
give options for mitigation such as lowering speed, restricting parking, restricting turning
movements, etc. This information will be compiled into a report which can be utilized by your
office to present to the City Council for an ordinance to address the final mitigation efforts. We
have based our Fee Proposal on the aforementioned assumptions.

CrossRoad Engineers is pleased to present this Scope of Services and Not to Exceed Fee
Proposal to provide professional services associated with the construction of lighting and
signage at three intersection locations in Carmel, Indiana. These locations include 96` Street
and Ditch Road; 116 Street and Ditch Road; and 106` Street and Towne Road.
Our scope of services for this work will include topographic survey, development of construction
plans, project bidding, construction staking, inspection, and as -built plans. Per our conversation,
we will provide these services on an hourly basis per the attached billing rate sheet. Therefore,
we respectfully request a Not -to- Exceed purchase order in the amount of $20,000.
