ADDITIONAL SERVICES AMENDMENT TO
AGREEMENT FOR PURCHASE OF GOODS AND SERVICES
THIS AMENDMENT TO THE GOODS AND SERVICES AGREEMENT ( "Agreement") entered into by and
between the City of Carmel and Crossroad Engineers, P.C. (the "Vendor "), as City Contract No. 030310,03 dated
March 3, 2010 shall amend the terms of the Agreement by adding the additional services to be provided by Vendor
consistent with the Scope of Work attached hereto and incorporated herein as Exhibit "A ". Furthermore, the terms of
the Agreement shall be amended to include the E- verify requirement as stated in Indiana Code § 22 -5-1.7 et seq.,
which is attached hereto and incorporated herein as Exhibit "B ", as well as the required E- verify Affidavit, attached
hereto and incorporated herein as Exhibit "C ". The terms and conditions of the Agreement shall not otherwise be
affected by this Additional Services Amendment and shall remain in full force and effect.

As we discussed, the City of Carmel has a need for additional professional services and pursuant to our current
contract with Crossroad Engineers, Contract #030310.03 dated March 3, 2010, 1 have requested and received from
you the proposed scope of services and fee estimates for the following work task:

Per our contract procedural requirements, 1 hereby approve these Additional Professional Services estimates, and
request that the Board of Public Works ratify the above referenced Contract Amendment.
PLEASE NOTE
Please be advised that "Additional Services" performed on City contracts must be billed separately from the original
contract and other additional services. More importantly, the invoice must reference the appropriate Additional
Service # and P.O. # shown above. Invoices received without the Additional Service # referenced will be returned
for proper identification. Thank you for your continued assistance in providing the City of Carmel with your

RE: On -Call Plan Review
Supplemental Request
Dear Mr. McBride.
We have prepared this proposal to continue providing professional services for the City of
Carmel. CrossRoad Engineers will provide these services as an extension of the City Engineer's
staff on an hourly basis.
The specific services which we expect to provide include technical reviews of developer -
prepared plans and other various miscellaneous engineering and surveying services as your
office requests.
Attached are our 2012 Hourly Billing Rates which we will be using for the work the remainder
of this year. Per our projected work load with submitted plan reviews, we would suggest adding
an additional amount of $35,000 to the contract ceiling against which we would invoice. Due to
recent increase in submittals, it is expected that this amount will serve for billing periods of July
through December, 2012. We will continue to work as efficiently as possible to minimize the
City's expenditures on these developer driven projects, and hopefully the requested amount lasts
well into 2013. At such time as the supplemental amount is depleted, we shall contact you
immediately.
We appreciate the City's trust in our ability to provide these services and look forward to
continue working closely with your staff and helping you keep the ever- growing list of projects
moving through your office. If you should have any questions or need any further information.
