ADDITIONAL SERVICES AMENDMENT TO
AGREEMENT FOR PURCHASE OF GOODS AND SERVICES
THIS AMENDMENT TO THE GOODS AND SERVICES AGREEMENT ( "Agreement ") entered into by and
between the City of Carmel and First Group Engineering, Inc. (the "Vendor"), as City Contract dated August 1, 2012
shall amend the terms of the Agreement by adding the additional services to be provided by Vendor consistent with
the Scope of Work attached hereto and incorporated herein as Exhibit "A ". Furthermore, the terms of the Agreement
shall be amended to include the E- verify requirement as stated in Indiana Code § 22 -5 -1.7 et seq., which is attached
hereto and incorporated herein as Exhibit "B ", as well as the required E- verify Affidavit, attached hereto and
incorporated herein as Exhibit "C ". The terms and conditions of the Agreement shall not otherwise be affected by this
Additional Services Amendment and shall remain in full force and effect.

SUMMARY
The proposed work items below are for Alternate 2 from Appendix 9 of the Williams Creek Watershed
Master Plan.
The proposed work would include:
• Topographical Survey of project area
• Approximately 900 feet of vertical realignment of Spring Mill Road.
• Approximately 200 feet of vertical realignment of the private drive just south of the unnamed tributary east
side of Spring mill Road.
• Twin 8'X6" concrete culverts under Spring Mill Road
• Soils Investigation for the Design of twin 8'X6" culverts under Spring Mill Road with subgrade and
pavement design considerations.
• Construction Plans including items shown on the Man -hour Justification
• Maintenance of Traffic (MOT) Plans using Detour
• Utility Coordination
• Contract Documents
• Preparation of Water Permits using information as provided in the Master Plan (IDEM 401, Rule 5, Corps
404 and IDNR Construction in a Floodway and possibly County Regulated Drain)
Does not include:
• Stream Modeling, drainage or detention design calculations
• Determination of flood easements, wetland delineation or Environmental Mitigation
• Route Survey Plat
• Addition of sidewalks /bicycle facilities
• Street Lighting
• Plans for MOT phase construction
