We anticipate providing the successful Contractor with the parameters by which they are authorized to work within the existing right of way. Any survey necessary for the Contractor to prosecute the work or stay within the provided right of way shall be the responsibility of the Contractor. Scope of Services to be provided by United 1. United shall contract with ATC for the preparation of the geotechnical recommendations necessary for the design of the sheet pile wall by the Contractor and preparation of the proposed structure subgrade. Based upon the schedule, we do not anticipate having time for the recommendations to be prepared and the sheet pile wall designed before it is necessary for the quote solicitation. The RFQ document will provide the limits for the sheet pile wall along with the anticipated thickness and quantities. 2. The existing pipe inflow to the sewer box is a 66 inch CMP pipe. United is proposing to replace the existing 5' x 5' box culvert outflow with a 66 inch smooth concrete pipe, matching the existing inflow pipe in-kind. The hydraulic properties of the proposed pipe will be greater than the existing pipe due to the difference in pipe type. 3. United will develop a RFP document to define the work required and form the basis for the contractor quotes. We anticipate using 10 or less pay items to define the project. Within the RFP, United will provide Exhibits which will define how the quantities for the pay items were calculated. These Exhibits will aid in determining changes required to the contractor payment during construction. 4. United will answer bidder questions during the process and will compile and evaluate the bids. 5. Once the project has been awarded, United proposes that our Post Bid Construction involvement includes answering construction questions, reviewing and approving submittals and providing a part time Construction Inspector as needed and requested by the City. We have proposed this portion be administrated on an hourly basis. Scope of Services to be provided by City of Carmel 1. The City of Carmel shall assist in coordinating the proposed "emergency" project with all permitting agencies. United will send an initial notification and continue through the coordination process for obtaining the IDEM 401, the USACOE 404, and the Hamilton County Crossing Permit. 2. The City of Carmel shall assist with coordinating the proposed parameters of the closure of East 96th Street with any affected parties, including the City of Indianapolis. United will send an initial notification and continue through the coordination process. 3. United will send out initial coordination letters with area utilities, the City of Carmel shall assist United and the successful contractor with completing the utility coordination due to the schedule of the proposed project. East 96th Street and College Avenue October 18, 2012 Page 3 of 3 4. City of Carmel shall assist United in providing right of entry documentation for inclusion in the RFP document. Schedule 1. The City of Carmel desires the project to be completed before the winter. United shall target November 30, 2012 as the project completion date for planning purposes, as this is the typical date for the asphalt plant shutdown. 2. United shall provide the RFP document by October 29, 2012. Please find below our proposed lump sum fees for the Scope of Services defined above.
