AGREEMENT FOR PURCHASE OF GOODS AND SERVICES
THIS AMENDMENT TO THE GOODS AND SERVICES AGREEMENT ("Agreement") entered into by and
between the City of Carmel and First Group Engineering, Inc. (the "Vendor), as City Contract dated October 11,
2011 shall amend the terms of the Agreement by adding the additional services to be provided by Vendor consistent
with the Scope of Work attached hereto and incorporated herein as Exhibit "A". Furthermore, the terms of the
Agreement shall be amended to include the E-verify requirement as stated in Indiana Code § 22-5-1.7 et seq., which
is attached hereto and incorporated herein as Exhibit "B", as well as the required E-verify Affidavit, attached hereto
and incorporated herein as Exhibit "C". Also, pursuant to I.C. § 5-22-16.5, Vendor shall certify that in signing this
document, it does not engage in investment activities within the Country of Iran. The terms and conditions of the
Agreement shall not otherwise be affected by this Additional Services Amendment and shall remain in full force and
effect.

contract with First Group, dated October 11, 2011. I have requested and received from you the proposed scope of
services and fee estimates for the following work task:

Please be advised that "Additional Services" performed on City contracts must be billed separately from the original

Dear Mr. McBride:
Due to the revisions to the horizontal and vertical alignment, structure, path and lack of
hydraulic information from the Christopher Burke stream modeling, additional work is
proposed for this project.
Please find attached our summary of the proposed added work, estimated design costs
and man-hour justification breakdown. Upon your approval we will finish the plans and
prepare a sub-agreement with USI for the stream modeling so they can complete the
hydraulic analysis for the permitting.

SUMMARY
The proposed work items below are for a supplemental for realigning Spring Mill Road to minimize right of
way taking,additional channel survey for stream modeling of the tributary under Spring Mill Road,stream modeling
for the tributary under Spring Mill Road and adding an off roadway multi-use path along Spring Mill.
The proposed work would include:
• Additional Topographical Survey up and down stream of the proposed Structure for stream modeling
• Approximately 890 feet of horizontal and vertical realignment of Spring Mill Road and add path
• Approximately 200 feet of vertical realignment of the private drive just south of the unnamed tributary east
side of Spring mill Road.
• Revise location of concrete culvert under Spring Mill Road
• USl to prepare stream modeling for tributary
Estimated Design Costs:
First Group Engr:
Plan revisions and survey $10,500.00
USI Consultants:
Stream modeling, structure size and type
and coordination with Christopher Burke(CB) $6,500.00
Coordination with County for determination of floodway storage (if requested by City) $1,000.00
Additional coordination with the County $1,000.00
If base model is NOT provided by CB (if needed basis) $3,000.00
TOTAL $22,000.00
